Background to the cCNS pathway The Policy on the Development of Graduate to Advanced Nursing and Midwifery Practice (DOH, 2019) outlines a pathway to support a change for specialist practice to develop and meet service need, including the revised eligibility criteria for CNS as follows: · A minimum of 1 years’ experience in practice prior to completing the required post graduate qualification in the specialist area at level 9 (equivalent to 60 ECTS or above) · Recognition as a CNS to meet service needs therefore requires the completion of a postgraduate qualification together with a minimum of 1 years’ specialist experience · This is in addition to meeting the requirements set by the HSE to ensure the delivery of safe, effective practice to meet service demands In response to an identified service need a candidate CNS pathway has been established and sets out the following: Nurses currently employed as Staff Nurses (SN) and Enhanced Nurses (EN) and any other nursing grades who have a minimum of 1 years clinical experience and who express an interest in CNS roles could be supported to progress on a candidate CNS pathway as follows: (a) A recruitment campaign will be organised by the CHO/HG between local HR and services. The above pathway is valid from May 2023 to September 2026 Details of Service Cork University Hospital (CUH) is the largest acute hospital in the country. The hospital is a primary teaching hospital with close alliances to University College Cork (UCC), nursing, medical and allied health professional training. The hospital also plays a central role in the management of co-located services on site. It is the only Level 1 trauma centre for the Republic of Ireland – an accreditation given because of the very wide range of specialities delivered by the hospital – including Neurosciences, Cardiac Services, Orthopaedics, General Surgery, Renal, Internal Medicine, Vascular, Ophthalmology, Urology, Plastic Surgery, Maxillary-Facial, Paediatrics, Intensive Care, Oncology, Haematology, Obstetrics, Gynaecology and Neonatology and Emergency Medicine. CUH is the tertiary referral centre for the HSE Southern area, and the supra regional area of Limerick, Clare, Tipperary, Waterford and Kilkenny. CUH therefore acts as a regional centre for secondary and tertiary care for the catchment population of 550,000 served by the HSE Southern area and a supra-regional centre for a total a population of 1.1 million. In 2019 CUH had 69,982 presentations to the Emergency Department, 222,115 outpatient attendances and 31,769 inpatient discharges. CUH currently employs 3,488.26 (WTE) staff of multiple professions and is the primary teaching hospital for the Faculty of Health and Science in University College Cork. UCC is the Academic partner of the South-South West Hospital Group. Cork University Hospital has very strong relationships with each of the six schools within the Science Faculty of UCC and this is a key area for future development to maximise the opportunities for both the service and academia. Reconfiguration of Services Cork & Kerry The Reconfiguration roadmap was published on the 22nd November 2011. It outlines a plan of how, why and where we deliver our acute hospital services. The roadmap provides for the first time in this region, a plan that has been agreed between all six acute hospitals on how services should be re-organised in order to ensure the best possible healthcare system for people in Cork and Kerry. Care is delivered in a variety of settings and new approaches and models of care to follow the care demands of the health service are being advanced within the context of overall healthcare reform, to include Slaintécare. This role has been created as a component of delivery of the Headache Pathway. Headache is one of a number of modernised care pathways developed by the HSE. The Headache Pathway will provide an integrated approach to the management of headache in the community and via tele-medicine. The Early implementation community-based model includes the input of Nurse Led Clinics, additional services for those requiring support in self-management and ongoing support for general practitioners who provide the key link between community and the acute hospital. These nurse led clinics can be developed as tele-linked services, with the capacity to provide expert care that will combine tele-health linked services with clinic attendance. Targets have been established for each pathway and KPIs will be measured, with strong expectation that these targets are achieved. These outputs will be agreed locally with the Headache implementation group and will focus primarily on a reduction by 25% of those patients on the waiting list complaining of headache. A service audit of their activity will also be incorporated into the job descriptions for the Headache Pathway roles and an annual report will be required. Purpose of the Post As outlined in this job description, the cCNS pathway will facilitate the post holder to be supported to professionally and clinically develop the skills and knowledge required to achieve the competencies of the CNS role. The cCNS post holder will be supported to acquire the necessary knowledge and skills required to deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse/ Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008. The overall purpose of the candidate Clinical Nurse Specialist Neurology (Headache) post is to be support the candidate to develop the skills and knowledge required to provide safe, timely, evidenced based nurse-led care to patients with Headache diagnosis at an advanced nursing level. This will involve acquiring the skills and knowledge required to undertake and document a complete episode of patient care (assess, diagnose, plan, treat and discharge patients) according to collaboratively agreed protocols and agreed scope of practice in the clinical setting. The candidate will be expected to acquire the necessary advanced clinical and theoretical knowledge, critical thinking and decision making skills to successfully undertake the role. This will be achieved through the completion of both clinical practice experience and/or relevant postgraduate education/training. To develop the skills and knowledge required to support a safe environment for patients with a Headache diagnosis by increasing the use of evidence based clinical guidelines, address patient expectations and promote wellness and evaluate care given. To be supported to develop the skills required to provide professional and clinical leadership in order to develop nurse led practice and health policy at local, regional and national level. To develop the skills and knowledge required to lead, conduct, disseminate and publish nursing research to shape and advance nursing practice, education and health care policy at local, national and international levels. The clinical practice role may be divided into direct and indirect care. Direct care comprises the assessment, planning, delivery and evaluation of care to the service user, family and/or carer. Indirect care relates to activities that influence and support the provision of direct care. Direct Care Throughout the agreed pathway, the cCNS/ will be enabled to develop the specific knowledge, skills and competencies to: · Provide a specialist nursing/midwifery service for service users with a diagnosis of aHeadache condition who require support and treatment through the continuum of care · Undertake comprehensive service user assessment to include physical, psychological, social and spiritual elements of care using best evidence based practice in the care of Headache . · Use the outcomes ofnursing/midwifery assessment to develop and implement plans of care/service user group management to contribute to the plans of service users, their families/carers and the MDT · Monitor and evaluate the service user’s response to treatment and amend the plan of care accordingly in collaboration with the MDT and service user, family and/or carer as appropriate. · Make alterations in the management of service user condition in collaboration with the MDT and the service user in line with the nursing/midwifery aspect of agreed pathways and policies, procedures, protocols and guidelines (PPPG’s). · Accept appropriate referrals from MDT colleagues · Co-ordinate investigations, treatment therapies and service user follow-up · Communicate with service users, family and/or carer as appropriate, to assess service user needs and provide relevant support, information, education, advice and counselling as required · Where appropriate, work collaboratively with MDT colleagues across Primary and Secondary Care to provide a seamless service delivery to the service user, family and/or carer as appropriate Participate in medication reconciliation taking cognisance of poly-pharmacy and support medical and pharmacy staff with medication reviews and medication management Identify and promote specific symptom management strategies as well as the identification of triggers which may cause exacerbation of symptoms. Provide service user with appropriate self-management strategies and escalation pathways Manage nurse led Headache clinics in collaboration with the MDT Identify health promotion priorities for the service user, family and/or carer and support service user self-care in line with best evidence. This will include the provision of educational and health promotion material which is comprehensive, easy to understand and meets service user’ needs Indirect Care Throughout the agreed pathway, the cCNS will be enabled to develop the specific knowledge, skills and competencies to: · Identify and agree appropriate referral pathways for service user with a headache condition · Participate in service user case reviews with MDT colleagues · Use a case management approach to service user complex needs in collaboration with MDT in both Primary and Secondary Care as appropriate · Take a proactive role in the formulation and provision of evidence based PPPGs relating to Headache care · Take a lead role in ensuring the nursing service for service user with a headachecondition is in line with best practice guidelines and the Safer Better Healthcare Standards (HIQA, 2012) Service user/Client Advocate Throughout the agreed pathway, the cCNS will be enabled to develop the specific knowledge, skills and competencies to: · Communicate, negotiate and represent service user, family and/or carer values and decisions in relation to their condition to MDT colleagues in both Primary and Secondary Care as appropriate · Develop and support the concept of advocacy, particularly in relation to service user participation in decision making, thereby enabling informed choice of treatment options · Respect and maintain the privacy, dignity and confidentiality of the service user, family and/or carers · Establish, maintain and improve procedures for nursing/midwifery collaboration and cooperation between Acute Services, Primary Care and Voluntary Organisations as appropriate · Proactively challenge any interaction, nursing/midwifery or otherwise, which fails to deliver a good quality service to service user. Education & Training Throughout the agreed pathway, the cCNS will be enabled to develop the specific knowledge, skills and competencies to: · Develop clinical competence in service user management within the area of headache keeping up-to-date with relevant research to ensure the implementation of evidence based practice. · Provide the service user, family and/or carer with appropriate information, education and other supportive interventions to increase their knowledge, skill and confidence and autonomy in managing their Headache condition. · Contribute to the design, development and implementation of education programmes and resources for the service user, family and/or carer in relation to a headache to enable them to manage their own condition. · Participate in training programmes for nursing/midwifery, MDT colleagues and key stakeholders as appropriate · Create exchange of learning opportunities within the MDT in relation to evidence based headache nursingdelivery through journal clubs, conferences etc. · Develop and maintain links with Regional Centres for Nursing & Midwifery Education (RCNMEs), the Nursing and Midwifery Planning and Development Units (NMPDUs) and relevant third level Higher Education Institutes (HEIs) in the design, development and delivery of nursing/midwifery educational programmes in headache care · In tandem with the line management structure, be responsible for addressing own continuing professional development (CPD) needs to achieve competencies required for the role · Use agreed protected time for research, education and professional development · With the line manager, use the Professional Development Planning Framework for Nurses and Midwives to plan and self-assess competency achievement and additional CPD needs Audit & Research Throughout the agreed pathway, the cCNS will be enabled to develop the specific knowledge, skills and competencies to: · Establish and maintain a register of service user with a headache condition within the cCNS Caseload. · Maintain a record of clinically relevant data aligned to National Key Performance Indicators (KPI’s) as directed and advised by the DoN/M. · Identify, initiate and conduct nursing/midwifery audit and research relevant to the area of practice and take part in MDT audit and research. · Identify, critically analyse, disseminate and integrate into practice, best evidence relating to care of headache conditions · Contribute to nursing research on Headache care. · Use the outcomes of audit to improve nursing/midwifery service provision and advocate, when appropriate, for improvement of non-nursing/midwifery services · Contribute to service planning and budgetary processes through use of audit data and specialist knowledge · Monitor, access, utilise and disseminate current relevant research to advise and ensure the provision of informed evidence based nursing/midwifery practice Audit expected outcomes including · Collate relevant data (insert agreed KPIs/clinical targets) which will provide a baseline audit of expected outcomes and demonstrate evidence of the effectiveness of the cCNS interventions undertaken and refer to the National KPIs associated with the specialty. CUH is the tertiary referral centre for the HSE Southern area, and the supra regional area of Limerick, Clare, Tipperary, Waterford and Kilkenny. CUH therefore acts as a regional centre for secondary and tertiary care for the catchment population of 550,000 served by the HSE Southern area and a supra-regional centre for a total a population of 1.1 million. In 2019 CUH had 69,982 presentations to the Emergency Department, 222,115 outpatient attendances and 31,769 inpatient discharges. CUH currently employs 3,488.26 (WTE) staff of multiple professions and is the primary teaching hospital for the Faculty of Health and Science in University College Cork. UCC is the Academic partner of the South-South West Hospital Group. Cork University Hospital has very strong relationships with each of the six schools within the Science Faculty of UCC and this is a key area for future development to maximise the opportunities for both the service and academia. Reconfiguration of Services Cork & Kerry The Reconfiguration roadmap was published on the 22nd November 2011. It outlines a plan of how, why and where we deliver our acute hospital services. The roadmap provides for the first time in this region, a plan that has been agreed between all six acute hospitals on how services should be re-organised in order to ensure the best possible healthcare system for people in Cork and Kerry. Care is delivered in a variety of settings and new approaches and models of care to follow the care demands of the health service are being advanced within the context of overall healthcare reform, to include Slaintécare. As outlined in this job description, the cCNS pathway will facilitate the post holder to be supported to professionally and clinically develop the skills and knowledge required to achieve the competencies of the CNS role. The cCNS post holder will be enabled to deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse/Midwife Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008. The overall purpose of the candidate Clinical Nurse Specialist Neurology (Neuromuscular) post is to be support the candidate to develop the skills required to provide safe, timely, evidenced based nurse-led care to patients with Neuromuscular diagnosis at an advanced nursing level. This will involve acquiring the skills and knowledge required to undertake and document a complete episode of patient care (assess, diagnose, plan, treat and discharge patients) according to collaboratively agreed protocols and agreed scope of practice in the clinical setting. The candidate will be expected to acquire the necessary advanced clinical and theoretical knowledge, critical thinking and decision making skills to successfully undertake the role. This will be achieved through the completion of both clinical practice experience and/or relevant postgraduate education/training. To develop the skills and knowledge required to support a safe environment for patients with Neuromuscular diagnosis by increasing the use of evidence based clinical guidelines, address patient expectations and promote wellness and evaluate care given. To be supported to develop the skills required to provide professional and clinical leadership in order to develop nurse led practice and health policy at local, regional and national level. To develop the skills and knowledge required to lead, conduct, disseminate and publish nursing research to shape and advance nursing practice, education and health care policy at local, national and international levels. In 2021 (which was a Covid year), CUH had 69,087 ED presentations; 215,529 physical out-patient attendances; 50,173 virtual out-patient attendances; 29,966 inpatient discharges and 27,960 day cases. The main purpose of the post is to develop the job description and supporting documentation under the direction of the Health Care Provider’s Advanced Practice Stakeholder Governance Group, to enable the individual nurse to meet the NMBI Criteria for Registration as an Advanced Nurse Practitioner as set out in Advanced Practice (Nursing) Standards and Requirements (NMBI, 2017) . The individual will undertake the academic preparation and develop the clinical and leadership skills, competencies and knowledge required to meet the criteria to be registered as a RANP with NMBI. The scope of the cANP role must reflect the incremental development of expertise and as such, the cANP cannot deliver care as an autonomous practitioner. Furthermore, the cANP will develop and submit their personal portfolio and all other necessary documentation to NMBI in order to register as a RANP. The value of the nursing contribution as a distinct profession must be safeguarded and articulated in the development of new services led by advanced nurse practitioners, complementing rather than replacing current services delivered by doctors (NMBI 2017 p.9). The overall purpose of the post is to provide safe, timely, evidenced based nurse-led care to patients at an advanced nursing level .This involves undertaking and documenting a complete episode of patient care (assess, diagnose, plan, treat and discharge patients) according to collaboratively agreed protocols and scope of practice in the clinical setting; demonstrating advanced clinical and theoretical knowledge, critical thinking, clinical leadership and decision making skills. The advanced practice role demonstrates a high degree of knowledge, skill and experience that is applied within the nurse-patient/client relationship to achieve optimal outcomes through critical analysis, problem solving and accurate decision-making (NMBI, 2017). Central to this is the provision of quality care, a safe environment and processes for patients by the use of evidence based clinical guidelines that address patient expectations, promote wellness and evaluate care given. The role will provide clinical leadership and professional scholarship in order to develop nursing practice and health policy at local, regional and national level.
