Adult Day Services Coordinator
Overall Purpose of the Post:
The Coordinator will be responsible for leading the effective development and delivery of the day service using a person-centred approach and in line with New Directions. The successful candidate will lead a dedicated team in delivering quality standards of care and support to adults with physical, sensory and intellectual disabilities, using a person-centred approach and in line with New Directions.
Key Duties and Responsibilities:
• Ensure the delivery of appropriate and effective service to each Service Owner ensuring the highest standards of care, and in line with national policy.
• Contribute to the development of strategies for the implementation of New Directions and development of support services to meet the identified and emerging needs of Service Owners.
• To work in collaboration with the other members of the multidisciplinary team in relation to the planning and delivery of services including the development of person-centred planning.
• To participate in joint nursing/respite assessments with colleagues, where appropriate.
• To ensure that appropriate, accurate and up to date written records and reports regarding service delivery are in place and maintained in accordance with organisational policy.
• To manage the service resources including the budget allocation, staffing and buildings/equipment.
• Liaise closely with the HSE, community nursing and therapy staff and other key organisations in the course of ensuring the delivery of a person-centred service to each service owner.
• Coordinate staffing resource to ensure that duties, activities and programmes allocated to staff are carried out efficiently and effectively.
• Liaise closely with community nursing, therapy staff and other key organisations in the CHO6 region in the course of ensuring the delivery of a person-centred service.
• To plan, develop and implement the delivery of services, to include:
o Criteria for accessing services.
o Assessment & individual service planning process.
o Person centred planning approach in partnership with Service Owners.
o Advocacy development on group and individual basis.
o Access to mainstream/community-based opportunities.
• To ensure service delivery is kept up to date and maintained in line with current good practice in the disability sector.
• Conduct and implement audit/service quality monitoring systems (EFQM).
• To work closely with the Adult Services Manager in developing the service in the local area, including drafting service plans and service development proposals.
• Contribute to the planning and implementation of information systems.
• To take responsibility for the safety, health and welfare of staff and service owners and to manage risk within the service.
• To act as designated safeguarding officer, and complaints officer for the service.
• Develop and deliver training to staff and Service Owners as required.
• Collaborate with Service Owners, family, carers and staff in the development of care plans and risk assessments for each Service Owner.
• To manage staff assigned to the centre, including recruitment, development, training, discipline and performance in accordance with Enable Ireland policies and procedures.
• Support staff by completing personal development planning, supervision and individual performance management and reviews as required.
• To coordinate student placements, including recruitment and screening and ensuring appropriate placement and supervision.
• To ensure the appropriate recruitment, screening, deployment and supervision of volunteers.
• To ensure effective communication between staff, the rest of the service and with Service Owners, parents and carers and other appropriate bodies.
• Plan, organise and deliver staff training in relevant areas as required.
• Actively participate in the setting of team and organisational goals.
To minimise exposure to breaches of GDPR, strict compliance is required in the course of carrying out the duties of this job and working with others. This will include but is not limited to compliance with Enable Ireland’s suite of GDPR Policies & Procedures, attending all GDPR training sessions and ensuring personal responsibility for implementing safeguards and measures as directed.
Terms & Conditions:
Responsible to: Adult Services Manager, or delegated manager.
Probation: A probationary period of 6 months applies, wherein three probationary meetings will take place to review your performance and suitability for appointment. The probationary period may be extended or terminated for any reason at Enable Ireland’s discretion.
Salary: The current salary scale for this post is €48,624 - €58,837 pro rata per annum.
Annual leave: Annual leave entitlement is 30 days pro rata per annum and proportionately less for less than 12 months service.
Pension Scheme: Enable Ireland operates a contributory pension scheme which all employees may join on earlier of 1st July or 1st January following start date.
Medical: The successful candidate will be required to undergo a medical assessment.
Garda Clearance / Police Clearance:
These will be required for all prospective employees who will undertake relevant work or activities relating to children or vulnerable persons.
Sick Pay:
(If applicable)
All periods of sickness exceeding two days must be medically certified. Weekly medical certificates are required thereafter. The Company reserves the right to have you examined by its own Doctor after 3 months continuous sick leave. Upon completion of 6 months continuous service with the Company sick pay will be as follows:
Full pay less social welfare for the first 13 weeks of sickness in any 12 month rolling period and half pay less social welfare for a further (13) weeks of sickness absence in the same 12 month rolling period.
Redeployment:
In exceptional circumstances the organisation reserves the right to redeploy you to an alternative role that is suitable to your skills and experience.
