Fixed-Term Contract – 12 months
Salary € 36,029.00 per annum
Full-Time, 35 hours per week
Monday to Friday 9.00am- 5.00pm
Based in Cork

Role
To work as a part of the Depaul Homeless Health Team , providing case management support to homeless people and marginalised groups who have complex support needs in the area of their medical and physical wellbeing to ensure they have access to the appropriate medical care within the context of a case management approach. To ensure that the relevant supports are in place and to work towards positive outcomes for people experiencing homelessness
This role is to support those who are homeless and living with chronic health conditions in addition to broader social inclusion cohort e.g migrants, traveller community.


Person Requirements
A minimum of 2 years’ experience working in the homeless sector or another related field. At least one year of this working with and co-ordinating support plans for service users with complex needs. (Essential)
An understanding of why people become homeless or may be at risk of homelessness and the issues they present with.(Essential)
Experience of interagency working within the voluntary and statutory sectors (essential)
