Your role:

To work as a part of the Depaul Homeless Health Team, providing case management support to homeless people and marginalised groups who have complex support needs in the area of their medical and physical wellbeing to ensure they have access to the appropriate medical care within the context of a case management approach. To ensure that the relevant supports are in place and to work towards positive outcomes for people experiencing homelessness. This role is to support those who are homeless and living with chronic health conditions in addition to broader social inclusion cohort e.g migrants, traveller community.

Key Areas of Responsibility
  • Provision of assertive engagement support services across community, hospital and accommodation services to Inclusion Health clients experiencing homelessness with complex health and/or addiction needs
  • Provide assertive support to Inclusion Health clients from migrant background with complex health and/or addiction needs
  • To support the presenting needs of clients and manage robust, effective working relationships with external agencies and partners of all health services (mental health, community and hospital based) including Mercy University Hospital, outpatient services, community GPs, researchers, Cork City Council/accommodation providers, and other professionals in homeless and drug services.
  • To assist medical professionals and project workers to engage and co-ordinate health issues of clients in crisis or as part of an on-going care plan
  • To assist with crisis management in the community and within the hospital where required

Person Requirements
  • A minimum of 2 years’ experience working in the homeless sector or another related field. At least one year of this working with and co-ordinating support plans for service users with complex needs. (Essential) 
  • An understanding of why people become homeless or may be at risk of homelessness and the issues they present with. (Essential) 
  • Experience of interagency working within the voluntary and statutory sectors (essential) 
  • Experience of lone working or off-site working (Desirable)
  • Experience of working alongside health supports, such as hospitals, methadone provision services, primary care services, medical social work etc
  • A clear understanding of the case management approach
  • Be able to work from a solutions focused, creative approach.
  • Excellent communication skills - both verbal and written and electronic.
  • Ability to be flexible and work in a variety of settings, with a variety of teams.
  • An ability to (and understanding of how to) advocate on behalf of the service users
  • An understanding of the statutory and voluntary sector services available to homeless people with complex needs.
  • Lived experience of homelessness and/or related issues will be taken into special consideration
