Case Manager

We're looking for a compassionate and proactive Case Manager to join our dynamic team in Meath, where person-centred care is at the heart of everything we do.
About the Role:
The Case Manager works alongside adults living with Acquired Brain Injury (ABI), supporting them to lead meaningful lives and participate fully in their communities. This role is about empowering each person to build on their strengths, make choices about their daily lives, and engage in activities that matter to them – both at home and in the wider community.
The Case Manager partners with individuals within the catchment area to understand their personal goals, identify their needs, and co-create tailored support plans that promote independence, dignity, and inclusion.
The contract available is Permanent, Full-Time, 39 hours.
The salary will be based on the HSE 2022 Grade VIIpayscale, pro-rated based on hours worked.
What You'll Be Doing:
- Co-ordinate referrals by building strong links with hospitals, GP’s, HSE Teams, and disability services to ensure timely and effective support for individuals with an acquired brain injury.
- Collaborate with the HSE and other agencies to priorities cases, attend meetings, and advocate for person served and their families.
- Conduct assessments and develop case management plans tailored to each person’s needs, promoting independence and community reintegration.
- Support seamless transitions between hospital, rehabilitation, and home or community settings, ensuring no one is left behind.
- Provide education and training to carers and families, helping them prepare for and support their loved one’s return home.
- Act as a resource and advocate, offering guidance to external providers and representing individuals with an ABI in forums and service planning.
- Maintain accurate records and reports, contributing to service development and ensuring accountability to funders and stakeholders.
- Promote ABI awareness and build partnerships with community groups, voluntary organisations, and statutory services.
Who We're Looking For:
We are seeking a compassionate and proactive Case Manager who thrives in a collaborative, community-focused environment. You will be confident building strong relationships with families, carers and persons served. You will bring excellent communication skills, along with the ability to advocate for individuals and co-ordinate support plans. You will have a collaborative mindset and a flexible approach.
Must Haves:
- A relevant third-level qualification in the Human Services sector (e.g. Health & Social Care)
- Proven experience supporting individuals and families affected by disability in a community setting
- Strong communication skills—verbal, written, report writing, and presentation
- Excellent attention to detail and sound judgement with a practical, hands-on approach
- Effective time management and a strong focus on client service
- Confident self-starter who thrives in a dynamic, fast-paced environment
- Skilled in conflict resolution and problem-solving
- High standard of record keeping and documentation
- Full clean driving licence
Great to Have:
- Previous experience working specifically with individuals affected by brain injury
