Clarecare is a professional social enterprise with charitable status, providing a range of people centred social services to individuals and families in County Clare for over 58 years. Current services provided include Family Support, Elderly Care Services, Counselling and Bushypark Residential Addiction Treatment Centre.  Clarecare’s headquarters are based in Ennis with local offices in Bushypark, Shannon, Kilrush, Killaloe and Ennistymon.

JOB DESCRIPTION

TITLE: Clinical Nurse – Home Support Service – East Clare

Full-time, Permanent role

DESCRIPTION OF ROLE:

We are expanding our Home Support Team with this additional role and therefore, require a motivated, dynamic and organised person who will be responsible for providing clinical guidance to front-line staff, Home Support Area Managers and other service areas within Clarecare. The successful candidate must be able to prioritise their own workload according to demand and also be somebody with a keen eye for detail. Please refer to full outline of position below.

ROLE: 

The Clinical Nurse is responsible for supporting best practice care to all the service users in the designated area. S/he will provide clinical guidance to front-line staff and Home Support Area Managers where appropriate to foster a clinical learning environment. They will provide and promote a model of care which is person centred, community focussed and will have an appreciation of the needs of the carer. They will be responsible for supporting and guiding all of the staff required to deliver the service through continued professional development by keeping up to date with research and new developments in Disabilities and Older Persons Services.

JOB RELATIONS:

The Clinical Nurse will work as part of the wider team in Clarecare and will report to the Home Support – Clinical Lead or designate. You may be required to work outside of normal hours if necessary. Flexibility and willingness to travel throughout Co. Clare is essential.

HOURS OF WORK:

Monday to Friday from 9.30 a.m. - 5pm daily with 30 mins. paid break per day. This position may involve evening or weekend work (as required) and flexibility is required regarding your work hours.

ANNUAL LEAVE:

The annual leave entitlement for this role is 26 days annual leave per full leave year (i.e. Jan-Dec inclusive). This excludes Public Holiday entitlement.

JOB LOCATION:

Remote/Hybrid working does not apply to this role. This is an onsite/community based role. The office base for this position will be Clarecare’s office, Killaloe, Co. Clare. The successful candidate will be required to travel within Co. Clare to Clarecare’s other Centre locations and to Service Users in the community. Therefore, flexibility and willingness to travel is required. You will be required to use your private car to travel for work purposes within the community and to other Clarecare Centre locations and must indemnity Clarecare on your private motor insurance policy in respect of same.

PURPOSE OF ROLE:

The purpose of the role is to ensure Clarecare’s Home Support Service is fully compliant in the areas of quality, care practice, governance and health & safety regulations.

SALARY & BENEFITS

  • The maximum gross salary per annum commensurate with relevant experience is €49,710 for this full-time, permanent position. In addition, you will receive (pending receipt of additional funding) a 2% salary increase on 1st April 2026 and a final 2% salary increase on 1st Oct 2026.
  • This however, should not be interpreted as providing for any additional salary increases beyond the above stated terms as Clarecare is a part-funded Section 39 entity and not a fully funded (Section 38) Agency.
  • 26 days annual leave per annum for full-time staff, excluding Public Holiday entitlement.

RESPONSIBILITIES OF THE POSITION:

General Responsibilities:

  • Ensure a high standard of professional and clinical skills to effectively serve Service Users receiving Home Support.
  • Ensure the highest professional standards of care are delivered using an evidence based, care planning approach.
  • Maintain the coordination, assessment, planning, delivery and review of Home Support Service Users across Co. Clare.
  • Provide safe, comprehensive nursing care to service users according to the Code of Professional Conduct as laid down by An Bord Altranais agus
  • Cnáimhseachais na hÉireann (Nursing Midwifery Board Ireland) and Professional Clinical Guidelines. Practice nursing according to: Professional Clinical
  • Guidelines, National and Area Health Service Executive (HSE) guidelines, Local / national policies, protocols and guidelines of current legislation.
  • Support, maintain, disseminate and review care practice policies, procedures and guidelines for the service.
  • Implement an audit portfolio with ongoing reviews to ensure all compliance obligations are met.·
  • Support the facilitation of all external audits within Clarecare e.g. HSE/HIQA and ensure all learnings/recommendations are shared and fully implemented.
  • Facilitate learning through continuing professional development courses as and when they arise.
  • Act as a professional/educational support resource for all staff and management in the areas of best practice Home Support services and clinical support. 
  • Respond to care practice issues and unanticipated events as arising.
  • Assist in developing and managing innovative pilot projects.
  • Prepare and disseminate materials for meetings/training sessions (Powerpoint presentations/Summary documents) and ensure accurate record keeping.
  • Represent Clarecare at relevant local/national fora. 
  • Carry out any other duties that may be assigned from time to time.
  • Provide clinical guidance to front-line staff and Home Support Area Managers where appropriate to foster a clinical learning environment.
  • Demonstrate clinical competence and professionalism.
  • Lead on practice development within the clinical area.
  • Promote, facilitate and support compliance with legal requirements as per Tender, policies and procedures affecting service users, staff and other Clarecare matters.
  • Be involved in the implementation of Home Support standards as and when issued by HIQA.
  • Ability to motivate staff and work to best practice and highest quality care standards.
  • In conjunction with the Clinical Lead and Compliance Service, develop and document quality and compliant care policies & procedures.
  • Ability to produce high quality audit reports, statistical information, research reports, etc
  • Investigate any accidents/incidents in the Home Support Service and implement recommendations across the Home Support Service.
  • Respect the rights, dignity and confidentiality of all Service Users that utilise our services and their carers/advocates.
  • Be a main contributor to clinical decisions within Clarecare Home Support Service.
  • CommunicationsExcellent communication skills both verbal and written (including writing complex reports).
  • Can communicate effectively with staff and managers in order to resolve any issues relating to care practice.
  • Ability to communicate at all levels of organisation – Senior Manager, Middle Management, and front-line staff.
  • Ability to build effective working relationships both internally and externally.
  • Ability to appropriately represent the service when necessary.
  • Ability to provide phone support and relay information to family carers on issues arising/feedback received from support staff in the daily care of a Service User in the service.
  • Network/ liaise with other service providers, health professionals and potential referral sources e.g. PHN’s, GP’s etc to promote the service.

  • Engage in continuing professional development by keeping up to date research and new developments in Gerontology or Older Persons Services & Disability Services and to attend staff study days as considered appropriate.
  • Can consistently maintain high standards of behaviour and performance, exercising due care and discretion, ensuring necessary and sufficient controls are in place to safeguard the integrity and reputation of Clarecare.
  • Ability to plan and work efficiently to establish priorities and allocate time and effort accordingly.

  • To maintain appropriate/accurate records of clients in receipt of the Clarecare service.
  • To be flexible and adaptable in one’s approach to work with service users and Home Support Workers. The work may include irregular hours and occasional weekend work.

PERSON SPECIFICATION:

Be registered on the Register of Nurses maintained by An Bord Altranais agus Cnáimhseachais na hÉireann (Nursing and Midwifery Board of Ireland) or be entitled to be so registered

And

Have at least 3 years post registration experience of which 2 years must be in the related area of Gerontology or Older Persons Services.

And

Demonstrate evidence of continuing professional developmen

And

Have the requisite knowledge and ability including a high standard of suitability and clinical, managerial support and administrative capacity to properly discharge the functions of the role

Annual registration

Maintain live annual registration on theRegister of Nurses maintained by Bord Altranais agus Cnáimhseachais na hÉireann (Nursing and Midwifery Board of Ireland) and provide current certification to Clarecare annually.

Other Essential Requirements:

  • A positive ‘fit to work’ medical. A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the
  • office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.
  • Positive Garda vetting disclosure through Clarecare.
  • Two positive written references, from your current and most recent employer.
  • Use of own car & full clean driving licence including adequate insurance to cover the use of your own car for work and to indemnify Clarecare in respect of this.
