Reporting Relationship: Reporting to Monaghan Childrens Disability Network Manager.

Details of Service:

Under Progressing Chidlrens Disability Services, The CDNT currently provide services for children with complex needs aged 0-18yrs. We provides a range of therapy, diagnostic and assessment services. Services include Home Support, Respite Care Packages, Home Nursing Care Packages and afterschool supports.

Our Team model is an interdisciplinary approach and our team includes OTs, Physios, Nurses, Social Workers, Psychologists, Dietician and SLTs.

Purpose of the Post:

The post of CNM 2 has a pivotal role in service planning, co-ordinating, and managing activity and resources within the clinical area. The main responsibilities are: quality assurance, resource management, staffing and staff development, practice development, facilitating communication and professional / clinical leadership.

The purpose of this post is to provide clinical nursing interventions in the area of Behavioural Specialist Nurse to the children and their families availing of the services of Monaghan CDNT. Key roles of the CNMII Behavioural Specialist Nurse within the Monaghan Children’s Disability Network Team includes:

1. Provide interventions to children and their familes based on evidence based practice in relation to promoting positive behaviour
and emphasing every child is an active participant in their home and local community.

2. Provide parent trainings and group work with other members of the CDNT on a regular basis in areas including but not restricted
to Positive Parenting, Understanding Autism, Interacting and Playing with your child.

3. Access, plan, implement and evaluate positive behaviour support plans where required.

4. Ensure all interventions promote the rights of the child and are based on the holistic assessment of the child and their famileis
needs.

5. Demonstrate a high level of specialist clinical knowledge and skills in the area of positive behaviour support to effectively carry
out their duties as CNMII Behavioural Specialist.

6. Work as part of the interdisciplinary team members making and accepting referrals for children as necessary.

7. Liaise with Child and Adolescent Mental Health Teams and Child and Adolescent Mental Health of Intellectual Disability Teams.

8. Liaise and work with Respite and Home Support providers as appropriate in the adherence of Positive Behaviour Support Plans.

9. Carry out and be involved in screening/Intake and Assessment of Needs as required.

10. Act as key worker/poin of contact for the child and family in the context of the multi-disciplinary team and the child’s individual
programme of care.

11. Liaise with Preschool providers, Special & Mainstream schools, parent groups and voluntary bodies where appropriate.

12. Develop strategies to monitor and support families to manage tensions and difficulties arising from the child’s disability and to
seek to implement strategies to reduce the impact of the childs behaviour.

13. To play a key role along with the MDT in ensuring the child’s individual intervention plan is implemented by the Home Support
Workers.

14. To liaise with TUSLA as appropriate and as per Childrens First Legistlation be identified as a mandated reporter..

15. To liaise with schools, SENOs, NEPS, Betterstart Staff as required.

16. To participate and chair team meetings, case conferences, review meeting and IFSP meetings as required.

17. To monitor and review external placements.

18. Support the community Paediatrician Clinic for children with positive behavioural needs.

19. To maintain up to date records and adhere to File Management Policy

20. To participate in Service Planning and Development.

21. To update the National databases.

22. Visit children and their families in locations such as hospitals, clinics and in their homes.

23. Perform such other duties as may be assigned by the CDNM/Disability Manager/ADON.

24. The CNMII will need to demonstrate appropriate expert clinical knowledge, leadership, education and an ability to use
research/audit to develop practice and improve current knowledge base.

A close professional working relationship will be developed and required between:

  • The post holder and the multidisciplinary team.
  • The local Nursing Community – PHNs, Outreach Nurses, Acute hospitals and Maternity Services
  • Transition into other Community Services including Primary Care, Child and Adolescent Disability Mental Health Teams,
    Tusla and Pre-school and School Services.
  • The multiple services a child with complex and multiple needs may access and encounter.
  • DKIT and partner education services.

Principal Duties and Responsibilities:

Professional /Clinical

The Clinical Nurse Manager 2 (Behavioural Specialist) will

  • To innovate care and initiate new policies, procedures and guidelines using evidence based practice where possible and developing an evidence base where minimal information may exist.
  • To organise with children and their families; their clinical management, utilizing clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate changes and plans of care. This will include monitoring and evaluating the response to treatment through analysis and interpretation of clinical tests and investigations.
  • To monitor and evaluate clinical outcomes of patient centered care including the Integrated Family Support Plan, involving the patient and their carers wherever possible, to develop services and care delivery around this need.
  • To communicate patient’s conditions, verbally and in writing, to other members of the healthcare team and contribute to decision-making processes in patient management.
  • To involve young people in service development and evaluation.
  • To promote clinical excellence through clinical improvement, audit and practice development.
  • To ensure young people and families are informed and properly involved in decisions and progress regarding care and progress.
  • To be part of the clinical meetings within the Louth Disability Services when relevant to the patient case load and contribute as applicable.
  • Lead by using patient experience to develop future services relevant to this patient group needs.
  • Develop and extend an understanding of the inherent issues and challenges faced by young people with disabilities in today’s society, and lifestyle choices available to them.
  • To teach practical skills to patients and families as appropriate utilising a positive behavioural approach in areas including but not exclusive to Sleep Hygiene,Toileting Independence etc.

· Act as a resource for children & families’ attending all services.

· Provide support to children & their families in preparation for major transitions – Commencing pre-school, School, special school,
going for surgery, transfer to another team etc.

· Helping to give confidence & skills to parents in dealing with pre-school, school, acute services etc.

  • Developing education material, information handouts for parents and children on various procedures, application processes and other therapeutic modalities.
  • Providing education and information to staff, community and schools on children with disabilities and their presentations
  • To participate in local, national and international benchmarking.
  • Manage patient care to ensure the highest professional standards using an evidence based, care planning approach.
  • Provide a high level of professional and clinical leadership.
  • Be responsible for the co-ordination, assessment, planning, delivery and review of service user care by all staff in designated area(s).
  • Provide safe, comprehensive nursing care to service users according to the Code of Professional Conduct as laid down by the Bord Altranais agus Cnáimhseachais na hÉireann (Nursing Midwifery Board Ireland) and Professional Clinical Guidelines
  • Practice nursing according to:

· Professional Clinical Guidelines

· National and Area Health Service Executive (HSE) guidelines.

· Local policies, protocols and guidelines

· Current legislation

  • Manage own caseload in accordance with the needs of the post.
  • Participate in teams / meetings / committees as appropriate, communicating and working in co-operation with other team members.
  • Facilitate co-ordination, co-operation and liaison across healthcare teams and programmes.
  • Collaborate with service users, family, carers and other staff in treatment / care planning and in the provision of support and advice.
  • Communicate verbally and / or in writing results of assessments, treatment / care programmes and recommendations to the team and relevant others in accordance with service policy.
  • Plan discharge or transition of the service user between services as appropriate.
  • Ensure that service users and others are treated with dignity and respect.
  • Maintain nursing records in accordance with local service and professional standards.
  • Adhere to and contribute to the development and maintenance of nursing standards, protocols and guidelines consistent with the highest standards of patient care.
  • Evaluate and manage the implementation of best practice policy and procedures e.g. admission and discharge procedures, control and usage of stocks and equipment, grievance and disciplinary procedures.
  • Maintain professional standards in relation to confidentiality, ethics and legislation.
  • In consultation with CDNM and other disciplines, implement and assess quality management programmes.
  • Participate in clinical audit as required.
  • Initiate and participate in research studies as appropriate.
  • Devise and implement Health Promotion Programmes for service users as relevant to the post.
  • Operate within the scope of practice - seek advice and assistance from his / her manager with any cases or issues that prove to be beyond the scope of his / her professional competence in line with principles of best practice and clinical governance.

Health & Safety

The Clinical Nurse Manager 2 (Behavioural Specialist) will:

  • Ensure that effective safety procedures are developed and managed to comply with statutory obligations, in conjunction with relevant staff e.g. health and safety procedures, emergency procedures.
  • Observe, report and take appropriate action on any matter which may be detrimental to staff and/or service user care or well being / may be inhibiting the efficient provision of care.
  • Assist in observing and ensuring implementation and adherence to established policies and procedures e.g. health and safety, infection control, storage and use of controlled drugs etc.
  • Ensure completion of incident / near miss forms / clinical risk reporting.
  • Adhere to department policies in relation to the care and safety of any equipment supplied for the fulfilment of duty.
  • Liaise with other relevant staff e.g. CNS infection control Occupational Therapist regarding appropriateness for procurement.
  • Have a working knowledge of the Health Information and Quality Authority (HIQA) Standards as they apply to the role for example, Standards for Healthcare, National Standards for the Prevention and Control of Healthcare Associated Infections, Hygiene Standards etc and comply with associated HSE protocols for implementing and maintaining these standards.
  • Support, promote and actively participate in sustainable energy, water and waste initiatives to create a more sustainable, low carbon and efficient health service.

Education and Training

The Clinical Nurse Manager 2 (Behavioural Specialist) will:

  • Engage in continuing professional development by keeping up to date with nursing literature, recent nursing research and new developments in nursing management, education and practice and to attend staff study days as considered appropriate.
  • Be familiar with the curriculum training programme for student nurses and be aware of the clinical experience required to meet the needs of the programme.
  • Participate in the identification, development and delivery of induction, education, training and development programmes for nursing and non-nursing staff.
  • Provide support and supportive supervision to Clinical Nurse Manager 1 and front-line staff where appropriate.
  • Supervise and assess student nurses and foster a clinical learning environment.
  • Engage in performance review processes including personal development planning as appropriate.

Management

The Clinical Nurse Manager 2 (Behavioural Specialist) will:

  • To act as a resource, providing clinical leadership and facilitating professional development amongst the multidisciplinary team.
  • Develop with the wider MDT evidence based policies and procedures pertaining to positive behavioural support needs of children with disabilities an/or complex needs.
  • Demonstrate clinical reasoning and decision making.
  • Work closely with the Clinical Leaders and Practice Educators to provide dynamic and strong education to the clinical areas to meet the current care delivery needs.
  • The ability to facilitate children and family learning through forums such as group education, parent workshops and MDT groups.
  • Working with others to promote and encourage each person’s contributions toward achieving optimal and realistic patient goals.
  • To demonstrate safety, quality and effectiveness by promoting clinical improvement and service development at Community, hospital, national and international level.
  • Act up for CDNM and chair Team Meetings where required.
  • Provide the necessary supervision, co-ordination and deployment of nursing and support staff to ensure the optimum delivery of care in the designated area(s).
  • Manage communication at ward and departmental level and facilitate team building.
  • Provide staff leadership and motivation which is conducive to good working relations and work performance.
  • Promote a culture that values diversity and respect in the workplace.
  • Formulate, implement and evaluate service plans and budgets in co-operation with the wider healthcare team.
  • Manage all resources efficiently and effectively within agreed budget.
  • Lead on practice development within the clinical area.
  • Lead and implement change.
  • Promote, facilitate and participate in the development of nursing policies and procedures. Monitor as appropriate and lead on proactive improvement.
  • Contribute to the formulation, development and implementation of policies and procedures at area and hospital level.
  • Ensure compliance with legal requirements, policies and procedures affecting service users, staff and other hospital matters.
  • Manage and promote liaisons with internal / external bodies as appropriate e.g. intra-hospital service and the community.
  • Actively participate in the Nursing Management structure by ‘acting up’ when required.
  • Maintain all necessary clinical and administrative records and reporting arrangements.
  • Engage in IT developments as they apply to service user and service administration.

Age

The CDNT places an emphasis on Continued professional development and promote recognition of specialist clinical skills and advanced practice.

Other requirements specific to the post:

Access to appropriate transport is essential

The post roster may vary from 5/7 shifts to 12hr shifts depending on service need and prioritisation decisions by the management team in children and nursing services.

The post may be assigned to other service areas within nursing roles within Disability Services as service need changes and/or service priorities emerge.
