Clinical Nurse Manager II, Community GP Liaison

Purpose of the Role:

The post of Community Liaison Nurse has the pivotal role of communicator acting as a vital link between the Emergency Department, General Practitioners, and Community Care Services to provide continuation of patient-focused care.

The post-Holder will encompass a major management / clinical and educational focus, which comprises of the delivery, follow-up, and evaluation of care to patients and their families, the General Practitioners, and the Community Care personnel regarding patients’ attendance at CHI at Temple Street.

Essential Criteria:

• RCN/RGN with 5 years of post-graduate experience or RGN with 5 years of paediatric emergency experience
• Experience working with a multidisciplinary team
• Knowledge of Paediatric child protection and safeguarding services.
• Paediatric Emergency Nursing course or a willingness to undertake
