Clinical Nurse Specialist, Older Person

The CNM2/CNS will provide care and ongoing management of patients with conditions relating to old age and work as a key member of the multi-disciplinary team providing physical, psychological and emotional support to older people and their family during their inpatient admission. The CNM2/CNS will deliver care inline with the five core concepts of the role set out in the framework for the establishment of clinical nurse/midwife specialist posts, 4th ed. NCNM 2008.

The CNM2/CNS should liaise closely with all Clinicians involved in diagnosis, management and ongoing care of all patients referred to the older person services. He/she will liaise with other health care professionals involved in patient care within the hospital, with other health care agencies as appropriate and with community services. While being autonomous, the CNM2/CNS should be aware of his/her own scope of practice and the need to refer patients to specialist services. The post holder will have an important educational role with the inter-disciplinary team and will play a role in the development of quality initiatives.
