The purpose of this Clinical Nurse Specialist (Palliative Care) post is to Contribute delivery of an efficient and effective Specialist Palliative Care Service that meets the needs of all patients and desired outcomes in line with key national drivers – National Standards for Safer Better Healthcare (HIQA, 2021), National Clinical Programme Palliative Care and Palliative Care Services Development Framework (HSE 2009). Contribute to the improvement of the health care experience and outcomes for older persons residing in nursing homes attending the Emergency Department or an inpatient, the emphasis being on minimising the hospital length of stay, with the provision of post discharge follow up and support to the patient and staff in the nursing home. Support appropriate and timely reduction of Emergency Department attendance through the development of care pathways that support GP’s and healthcare professionals in assessment and management of palliative care for older persons in nursing homes. Provide support and education to the older person, carers and healthcare professionals. The CNS will act as a resource for and liaise with other service providers to ensure an integrated approach to care. The CNS will support the needs of patients and those of their families by providing information, advice and support. This will extend to bereavement support of families or individuals following the death of a patient. The Clinical Nurse Specialist Palliative Care is a key member of the Consultant led interdisciplinary team providing Specialist Palliative Care to patients with malignant and non-malignant in the acute hospital. They will deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008

Location

The assignment will be to Our Lady of Lourdes Hospital Drogheda & Louth County Hospital.
