The purpose of this Clinical Nurse Specialist (Palliative care for Nursing Home Outreach) post is to:

• Deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4thedition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008.

• The CNS will work as part of a multidisciplinary team who will be responsible for implementing the delivery of within the community healthcare network and community healthcare organisation.

• This post will also involve the core elements of the CNS post to include clinical audit and research.
