Clinical Specialist Speech & Language Therapist, Frailty Intervention Therapy Team
The Clinical Specialist Speech and Language Therapist (SLT) will work closely with his/her HSCP colleagues on the FIT Team to develop and deliver a specialist inpatient and outpatient (as required) communication and swallow service to the frail population presenting to the Mater Misericordiae University Hospital (MMUH) Emergency Department (ED). The Clinical Specialist SLT will oversee the development of the Care of the Older Person SLT service and will provide clinical input in other areas of the service as required. Within the role, he/she will:
- Identify and assess the frail population presenting to ED as part of the multi-disciplinary FIT team in MMUH in accordance with the mission, values and strategic plan of the organisation.
- Act as senior decision-maker, with recommendations regarding HSCP intervention requirements & need for hospital admission, where appropriate.
- Determine discharge plan and appropriate follow-up arrangements with relevant services.
- Alternate team leader role within FIT team, providing leadership and direction to the FIT team activities.
- Communicate effectively with other team members, as well as patients, relatives, medical and nursing colleagues.
- Analyse service demands and develop the FIT team with other HSCP members in line with evolving requirements.
- Contribute to the overall operation and development of the communication and swallowing service within the hospital in collaboration with the SLT Manager and other SLTs within the department.
