Job Purpose/Role

The role of the Discharge Co-Ordinator is to ensure, with the inter-disciplinary team members, efficient and effective patient flow process between admission and discharge within the hospital. He/she will facilitate prioritisation of investigations/procedures, in consultation with clinicians and relevant health care staff, as well as address other critical factors that may impact on decisions to discharge patients.

A key component of this role includes the further development of existing liaison between the hospital and our community partners (including Public Health Nurses, Director of Nursing in Nursing Home’s, Health Service Executive, Post-Acute Care). The post holder will have responsibility for reviewing patients from our catchment area and elsewhere who frequently present at our Emergency Department for admission, with a view to specific care planning to support admission avoidance or expedited discharge.
