CNM, Virtual Ward
The post of CNM2 has a pivotal role in service planning, co-ordinating, and managing activity relating to the Virtual ward. The main responsibilities are: quality assurance, resource management, staffing and staff development, practice development, facilitating communication and professional / clinical leadership. This role will essentially be responsible for a service that provides hospital-level care and monitoring to patients in their own homes, rather than in a hospital bed. It uses technology, such as digital monitoring devices and smartphones, to remotely manage patients who are medically stable but still require care and treatment, often as an alternative to hospital admission or for faster discharge. Patients remain under the care of a hospital team and can benefit from improved recovery, reduced stress, and lower infection risk in a familiar home environment.
