CNS Stroke

The role of the Clinical Nurse Specialist in Stroke Care is to provide for the care and on-going management of patients with stroke. The post holder will be required to work as a key member of the multi-disciplinary team in the hospital providing physical, psychological and emotional support to stroke patients and their families throughout their stroke disease trajectory. She / he may act as a liaison between Stroke Services, Primary Care Teams, Community Care Teams and other agencies. The post holder will work as part of the multi disciplinary team to achieve the strategic objectives of the Stroke Service.
