We are seeking a Community Access Facilitator to join our Adult Disability Day Services team. This is an exciting opportunity to play a key role in empowering adults with disabilities to build confidence, achieve personal goals, develop valued social roles, and become active participants in their communities.

Working within the principles of New Directions and a Human Rights Based Approach, you will support people to explore opportunities for friendship, education, employment, volunteering, advocacy and community participation, while ensuring their voices, choices and rights remain at the centre of everything we do.

About the Role

As a Community Access Facilitator, you will:

  • Support individuals to identify and achieve personal goals through person-centred planning.
  • Promote community inclusion, active citizenship and independence.
  • Assist people to access education, training, volunteering and employment opportunities.
  • Help individuals develop social networks, friendships and meaningful community connections.
  • Support self-advocacy, informed decision-making and positive risk-taking.
  • Work collaboratively with families, circles of support, multidisciplinary teams and community organisations.
  • Provide practical and personal support when required, always maintaining dignity, respect and choice.
  • Act as a key worker and help ensure high-quality outcomes for the people you support.

Essential Requirements

Applicants must have:

  • A relevant QQI Level 5 qualification (or equivalent) in Health & Social Care, Community Care, Community Development, Youth Work, Healthcare Support or another relevant healthcare discipline; or

Relevant experience working in community, social care, disability services, employment support or similar settings is desirable.
