We're looking for a compassionate and dedicated Community & Case Manager to join our dynamic team in Navan, Co. Meath , where person-centred care is at the heart of everything we do.

About the Role:

As a Community Services/Case Manager, you will play a vital role in supporting adults with an acquired brain injury to reclaim independence, dignity, and purpose. This dynamic role involves working directly with individuals across the catchment area, helping them participate fully in home and community life. Our Case Management programme is rooted in a biopsychosocial model, delivered through an interdisciplinary team approach that adapts to the resources available locally.

The contract available is Specified Purpose (maternity leave), Full-Time, 39 hours.

The salary will be based on the HSE 2022+ Grade VIIpayscale, pro-rated based on hours worked.

What You'll Be Doing:

  • Lead case management and care planning for individuals with ABI
  • Support clients in achieving independence and community integration
  • Co-ordinate with teams an agency to deliver joined-up services
  • Manage staff rotas, supervision, and service operations
  • Oversee budgets, payroll, and purchasing for the local service
  • Ensure compliance with organisational policies and quality standards
  • Promote community links and access to employment, leisure, and supports
  • Represent the organisation and contribute to service development

Who We're Looking For:

We’re seeking a compassionate, organised, and proactive professional with experience in case management and community-based rehabilitation. You will be confident working both independently and as part of an interdisciplinary team, with strong leadership skills and a commitment to person-centred care. You will be passionate about empowering those with brain injuries to live full, independent lives and thrive in a dynamic environment.

Must Haves:

  • A relevant third-level qualification in the Human Services sector
  • Proven experience working with individuals and families in a support or care setting
  • Strong communication skills – verbal, written, report writing, and presentation
  • Demonstrated ability to lead and develop staff teams
  • Skilled in building positive relationships and stakeholders
  • High attention to detail, sound judgement, and a practical, hands-on approach
  • Excellent time management and customer service focus
  • Self-motivated and confident working in a fast-paced environment
  • Experience in planning, budgeting, and forecasting
  • Proficient in MS Office, including advances spreadsheet skills
  • Capable of managing conflict and resolving problems effectively

Great to Have:

  • Experience working in the disability or illness sector
  • Familiarity with interdisciplinary team environment
