Temporary Consultant Physician in Geriatric Medicine for 12 months

The consultant geriatrician plays a central role in the identification, admission, oversight, and discharge planning for patients utilising the contracted post-acute rehabilitation beds at Teaghlach Uilinn Nursing Home. They will work closely with multidisciplinary teams (MDTs) across University Hospital Galway (UHG), Merlin Park, and community services to select suitable low-complexity rehabilitation patients and ensure they have clear, goal-oriented rehab plans. The geriatrician is responsible for confirming rehab potential, guiding referral decisions, contributing to weekly MDT reviews, and ensuring that therapy goals align with each patient’s rehabilitation progress.

Once admitted, the consultant geriatrician provides ongoing clinical oversight, including participation in weekly MDT meetings and review of patients at in-person ward rounds. They support communication with GPs, community teams, and families, and contribute to discharge documentation and continuity of care. Where required, the geriatrician collaborates on escalation decisions and ensures patients’ transitions back into the community are appropriate and well-coordinated. Their leadership is key in integrating hospital, community, and nursing home care within the framework of ICPOP and Sláintecare goals.
