Details of Service

Services provided are

The Dietitian will primarily assist in the delivery of a specialist and comprehensive dietetic service within the Level 3 and 4 Obesity/Bariatric in St. Columcille’s Hospital, Loughlinstown.

The department of Nutrition & Dietetics provides a services to a wide range of areas including Endocrinology, Bariatric Surgery, general Medicine, Cardiology, Stroke Services, Cardiac Rehabilitation, Medical Observation Unit.

The department has a very busy outpatients services. It is an enthusiastic and dynamic department that is very interested in keeping up to date with the latest research in evidence-based practice. The department is involved in student training for B placement and Catering placement.

Caseload will include outpatients Centre for Obesity Services and inpatient services. Inpatient services covering our acute wards. St Columcilles Hospital is a 110 bed facility serving populations primarily care of the elderly, stroke and orthopaedic rehab, cardiac rehab, and general medical.

Reporting Relationship

There will be a professional reporting relationship for clinical governance and supervision with the Dietetic Manager or another nominated person.

Purpose of the Post

The purpose of this post is to work with the Senior Dietetics team in ensuring the co-ordination, development and delivery of a quality, patient centred nutrition and dietetic service that is in keeping with best practice.

· To provide a clinical nutrition and dietetic service to clients at St. Columcille’s Hospital and ensure appropriate follow up as necessary.

· To support the development of nutrition related policies at St. Columcille’s Hospital.

· Involvement in the process of Nutrition Care Process Model and any other record/data collection processes.

· To provide education and training to staff on nutrition policy and nutrition screening and patient follow up materials at St. Columcille’s Hospital.

· To keep up to date with nutrition and dietetic knowledge, health and safety, hygiene training, fire training and any other necessary dietetic clinical support training etc.

Principal Duties and Responsibilities

Professional/ Clinical

Key Duties and Responsibilities

1. Delivery of specialist behavioural obesity interventions as part of a multidisciplinary Level 3 Service – including nutritional assessment using nutrition care process, evidence based nutrition care planning, dietetic support for patients using obesity pharmacotherapy, group and 1:1 nutrition counselling, food diary analysis and skills based interventions.

2. Dietary management pre- and post- bariatric surgery as part of Level 4 Bariatric Service – including delivery of pre-surgery screening / education groups, telephone support and delivery of dietetic component of post op MDT surgical clinics, and the use of low calorie diets in an in-patient rehabilitation setting using patient centred care.

3. Delivery of telehelath interventions including video consultations and telephone consultations as appropriate to the service and patient.

4. Liaise with the MDT in the planning of integrated patient care. This involves attending and presenting at weekly MDTs, flexible delivery of both face to face and remote clinics across the week, use of telehealth messaging systems for dynamic service delivery, and clinical meetings as appropriate, within the Endocrine Services.

5. Dietetic support to acute medical wards for patients under the care of Endocrinology services.

6. Delivery of cookery skills classes within the Level 3 Obesity Service.

7. Implement Hospital Group Standards, take part in quality assurance initiatives, and contribute to the development, monitoring and review of standards of patient care.

8. To participate in research projects, audits, and key performance indicators where appropriate.

9. To identify and address dietetic and nutrition educational needs of all medical, nursing, catering and other health personnel.

10. To keep abreast of current advances in relevant aspects of nutrition & dietetics by undertaking CPD activities e.g. attending appropriate study days, journal club, conferences etc. Maintaining CORU certification.

11. To work in close co-operation with the catering department to ensure the nutritional needs of patients on therapeutic diets are met.

12. To maintain records regarding patient’s treatment and to collect statistics in line with hospital nutrition and dietetic service policy.

13. To participate in staff health & wellbeing initiatives as appropriate.

14. Delivery of dietetic assessment and acute care interventions to in-patients in St Columcille’s Hospital.

15. Management of clinical caseload, including clinical prioritisation, to provide patients with high quality, evidence based nutrition care

16. Working alongside established MDTs delivering evidence based nutrition interventions to general medical patients, Care of the Older Person teams and Stroke rehab services, Cardiology and Gastroenterology.

17. Delivery of Cardiac Rehabilitation sessions, in St Columcille’s as per agreed timetable with Cardiac Rehab MDT.

18. Coordination with Senior Dietitians of enteral and parenteral feeding policies and pathways within the hospital in conjuction with nursing, pharmacy and stores.

19. Leading on implementation of MUST training and cyclical audits in the hospital in line with HSE policies on nutrition screening in acute care settings.

Education & Training

Education & Training

The Dietitian, Staff Grade will:

20. Have and maintain active registration with CORU.

21. Strive to maintain standards of practice and levels of clinical knowledge by participating in a minimum of 30 hours of continuous professional development initiatives and attend relevant courses as appropriate.

22. Be a member of professional groups and participate in relevant forums pertaining to clinical nutrition and dietetics.

23. Produce and evaluate nutrition education materials for patients and multidisciplinary teams.

24. Engage in career and personal development.

25. Participate in clinical supervision and mentoring.

26. Attend for regular performance achievement sessions.

27. Actively participate in the staff grade rotations to new specialities within the department, as required.

28. Attend and present at journal club and clinical meetings within the department.

29. Engage in the education of colleagues, student dietitians and other health professionals.

Health & Safety

The Dietitian, Staff Grade will:

30. Become familiar with and work in accordance with relevant HSE Policies, legislation and professional policies, guidelines and requirements to ensure safe practice and high standards of service delivery.

31. Work in a safe manner with due care and attention to the safety of self and others.

32. Maintain appropriate patient record details and statistics in accordance with hospital/health centre and departmental guidelines, HSE Metrics and Healthstats along with the Freedom of Information Act.

33. Be aware of risk management issues, identify risks and take appropriate action, report all adverse incidents and near misses.

34. Have a working knowledge of the Health Information and Quality Authority (HIQA) Standards as they apply to the role for example, Standards for Healthcare, National Standards for the Prevention and Control of Healthcare Associated Infections, Hygiene Standards etcand comply with associated HSE protocols for implementing and maintaining these standards as appropriate to the role.

35. To support, promote and actively participate in sustainable energy, water and waste initiatives to create a more sustainable, low carbon and efficient health service.

Administrative

The Dietitian, Staff Grade will:

  • Prepare patient progress reports, performance indicators or statistics as required
  • Maintain data and audit records, and participate in yearly Key Performance Indicators.
  • Contribute to service planning and development in their area of assignment and has the ability to prepare and present relevant information that will aid operational and strategic planning for future service development.
  • Maintain appropriate records in accordance with legal and local requirements.
  • Make efficient use of developments in Information Technology.
  • Maintain professional standards with regard to patient and data confidentiality.
  • Keep up to date with organisational developments within the Irish Health Service.

(a)

Candidates for appointment must:

I. Be registered, or be eligible for registration, as a Dietitian by the Dieteitians Registration Board at CORU

AND

II. Candidates must have the requisite knowledge and ability (including a high standard of suitability and professional ability) for the proper discharge of the duties of the office.

AND

III. Provide proof of statutory registration on the Dietitian’s Register maintained by the Dietitians Registration Board at CORU before a contract of employment can be offered.

2.Annual registration

(i)

Practitioners must maintain annual registration on the Dietitians Register maintained by the Dietitians Registration Board at CORU.

And

(ii)

Confirm annual registration with CORU to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).

3.Health

Candidates for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.

4.Character

Candidates for and any person holding the office must be of good character.

Tenure

The current vacancies available are Permanent Basic Grade, whole time.

The posts are pensionable. A panel may be created from which permanent and specified purpose vacancies of full or part time duration may be filled. The tenure of these posts will be indicated at “expression of interest” stage.

Appointment as an employee of the Health Service Executive is governed by the Health Act 2004 and the Public Service Management (Recruitment and Appointments) Act 2004 and Public Service Management (Recruitment and Appointments) Amendment Act 2013.

Working Week

The standard working week applying to the post is to be confirmed at Job Offer stage.

HSE Circular 003-2009 “Matching Working Patterns to Service Needs (Extended Working Day / Week Arrangements); Framework for Implementation of Clause 30.4 of Towards 2016” applies. Under the terms of this circular, all new entrants and staff appointed to promotional posts from Dec 16th 2008 will be required to work agreed roster / on call arrangements as advised by their line manager. Contracted hours of work are liable to change between the hours of 8am-8pm over seven days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement (Implementation of Clause 30.4 of Towards 2016).

Annual Leave

The annual leave associated with the post will be confirmed at Job Offer stage.

Superannuation

This is a pensionable position with the HSE. The successful candidate will upon appointment become a member of the appropriate pension scheme. Pension scheme membership will be notified within the contract of employment. Members of pre-existing pension schemes who transferred to the HSE on the 01st January 2005 pursuant to Section 60 of the Health Act 2004 are entitled to superannuation benefit terms under the HSE Scheme which are no less favourable to those which they were entitled to at 31st December 2004.

Probation

Every appointment of a person who is not already a permanent officer of the Health Service Executive or of a Local Authority shall be subject to a probationary period of 12 months as stipulated in the Department of Health Circular No.10/71.

Protection of Children Guidance and Legislation

Health & Safety

It is the responsibility of line managers to ensure that the management of safety, health and welfare is successfully integrated into all activities undertaken within their area of responsibility, so far as is reasonably practicable. Line managers are named and roles and responsibilities detailed in the relevant Site Specific Safety Statement (SSSS).

Key responsibilities include:

· Developing a SSSS for the department/service[1], as applicable, based on the identification of hazards and the assessment of risks, and reviewing/updating same on a regular basis (at least annually) and in the event of any significant change in the work activity or place of work.
