OVERVIEW

  • Job Title & Grade – Discharge Planner (CNM2/Senior HSCP)
  • Tenure – The current vacancy available is a permanent full-time position.
  • Hours of Work – Full time WTE – the allocation of these hours will be at the discretion of the Department Head and in accordance with needs of service
  • Location of Post - Peamount Healthcare, Newcastle, Co. Dublin
  • Working Model - Fully Onsite
  • Remuneration - *Current salary scale with effect from June 1st, 2026: €62,699 (point 1) to €79,227 (point 11 LSI).
  • Annual Leave - As per Health Service Executive (HSE)
  • Closing Date – September 3rd 2026
  • Proposed Interview Date - To be confirmed - At least 1 weeks’ notice will be provided. Shortlisted candidates are expected to make themselves available for a face-to-face interview.
  • Take Up Appointment - Successful applicant(s) must be available to take up the role within a reasonable timeframe
  • Reporting Relationship – Director of Rehabilitation

PURPOSE OF THE POST

The overall purpose of the role is to support all discharge pathways in consultation with the Patient Flow and Operations Lead, senior clinicians/ MDT and acute partners. The cross cover of all aspects of the patient flow pathway where required. The following two areas fall within the remit of the role

  • Discharge Planning - Safe and proactive planning of discharge from day of admission and the management of discharges for patients with more complex interim or long-term needs.
  • Bed management - Safe allocation of beds to patients from various sources on a prioritised

POINT ON THE SCALE

New appointees coming from the private sector will be placed on the minimum point of the scale (*exceptions may apply for the below grades, provided proof of recognised service is submitted by VOS)

  • *Clinical and Therapy Grades and Staff Nurses*: Incremental credit may be applied for recognised relevant service in Ireland and abroad (Department of Health Circular 2/2011) in the Civil Service, local authorities, public service, health service and other public service bodies and statutory agencies.
