There is currently 0.5 WTE permanent / part time vacancy available in North Kildare Children’s Disability Network Team, 2nd Floor, HSE Primary Care Centre, Maynooth Road, Celbridge, W23 YK24, Co. Kildare.

A panel may be formed as a result of this campaign for Dublin and Midlands from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled.

Details of the Service

Children’s Disability Teams support children with complex needs arising from a disability. The interdisciplinary team provides support within a family centred practice model recognising the key role families and significant others play in a child’s development.

The CNM2 will work as part of the interdisciplinary team supporting children 0-18yrs of age and their families and will:

• Contribute to assessment, planning, implementation and intervention to support the goals identified as part of Individualised Family Service Plan.
• Take a lead in liaison with acute hospitals and other service providers for children accessing supports across a number of services.
• Take an active role in the development and delivery of training and education to families and others
