1)The Role:

The St. Columba’s Children’s Disability Network Team 3 provides support to children and young people who have complex needs arising from a disability. CDNT3 works according to an interdisciplinary model of support underpinned by Family Centred Practice.

This is an exciting opportunity to join the Children’s Disability Team based in Crumlin. We cover the catchment area of Crumlin, Drimnagh, Walkinstown, Parnell Road and areas of Harold’s Cross. The team works across home and community settings. CDNT3 offers a variety of supports including universal supports, targeted supports and individualised supports.

The Senior Medical Social Worker will work in line with team service plans and objectives and within established professional standards, guidelines and policies.

The Senior Medical Social Worker will contribute to service development, and training while collaboratively working with families and the team.

Our building is spacious with ample parking and can be easily accessed via public transport.

The team includes Psychologists, Occupational Therapists, Speech and Language Therapists, Physiotherapists, Speech and Language Therapists and Therapy Assistants

2)The Role:

Children’s Disability Network Team (CDNT) in Chamber House working with children and young people with complex disability needs in the West Tallaght/Rathcoole/Newcastle area.

The successful applicant will be joining a large interdisciplinary team with 5 Occupational Therapists, 4 Speech and Language Therapists, 2 Early Years Nurse Practitioners, 3 Psychologists, and 4 Physiotherapists.

This post would be well suited to candidates who are passionate about working with children and their families, and enjoy working closely with other disciplines.

Details of the Role CDNT1

· The post is based on the Children's Disability Network Team, Bru Chaoimhin. This team works with children and young people with complex needs requiring an interdisciplinary/ transdisciplinary team model focusing on the ICF-CY adapted “f words” functioning, family, fitness, fun, friends and future. The team are flexible and dynamic and use evidence based practice and good practice to ensure best outcomes for service users. The team work across clinic, school, home and community settings. The clinic is based in Dublin City Centre and benefits from easy access to a wide variety of amenities, it is easily accessible by public transport and there is free parking available.
