The person appointed to this post will work as part of a multi-disciplinary team delivering a co-ordinated approach to client care. They will deliver a coordinated approach to service user care across primary care, disability, older persons and mental health services.

The core philosophy of these services is to provide individualised care programmes for service users, based on identified need following a psycho-social assessment.

Integrated Care Programme for Older Persons

The Integrated Care Programme for Older Persons (ICPOP) community specialist team (CST) is a specialist transdisciplinary[1] service primarily targeting and managing the complex care needs of the older person with multiple co-morbidities across a continuum of care. The overall aims of the service are to:

  • Provide a specialist geriatric opinion using a multidisciplinary approach to support older people with complex care needs.
  • Develop a person-centred care planning approach that supports robust and timely communication across care settings.
  • Support appropriate and timely reduction of Emergency Department (ED) attendance through the development of care pathways that support GPs and others in assessment of older people with escalating care needs.
  • Provide support and education to the older person, carers and healthcare professionals.

The Social Worker, as part of the ICPOP CST will work closely with the consultant geriatrician and ICPOP operational team lead to develop and implement appropriate care pathways while working with primary, community and acute care services.

ICPOP utilises a case management approach to assess and meet the complex care needs of frail older persons. All clinicians on the ICPOP CST are responsible for case management. This includes the completion of a comprehensive geriatric assessment with each patient in their own home / clinic as appropriate.

Primary Care

Primary care social work services provide short term episodic intervention and support to individuals, families, couples, groups, and communities based on social assessment of need. The post holder will be physically located within the relevant community healthcare network. Social work is an integral and core part of the primary care multi-disciplinary team structure, providing assessments, consent led interventions to service users living in the community healthcare network.

Disability Services

The aim of the Disability Service is to deliver a person-centred service underpinned by a social model of care. Upholding the values and ethics of our profession, social workers are committed to involving people with disabilities in the design and delivery of the services and the supports provided, whilst balancing the challenges presented by case complexities and areas of risk. The purpose of the post is to manage the provision of a social work service within a multidisciplinary context and in so doing, ensuring a person-centred service to individuals and families underpinned by current ethics, legislation and disability social care philosophies. The person appointed to this post will manage a social work caseload which will include a higher complexity of need and which requires high levels of experience and expertise. They will complete social work assessments and interventions as clinically indicated and will support the professional development and supervision of Professionally Qualified Social Workers, as required.

Safeguarding and Protection Team (SPT)

The Adult Safeguarding and Protection team provides a safeguarding service to individuals and organisations in accordance with the “Safeguarding Vulnerable Persons at Risk of Abuse National Policy and Procedures” 2014. This includes:

· Providing oversight of safeguarding concerns of abuse within organisations funded by the HSE Social Care Division for older people and persons with disabilities via the Online System for Adult Safeguarding (OSAS).

· Participate in the delivery of a social work led safeguarding duty service providing safeguarding support, advice and guidance to members of the public and professionals.

· Managing complex safeguarding cases in the community via direct social work support and case management.

· Contributing to and participating in safeguarding awareness events and training delivery.

Memory Assessment and Support Service

The MASS aims to provide an assessment, diagnostic and post-diagnostic service for patients with suspected presentations of dementia, under the clinical governance of a Consultant Geriatrician. This specialist service has a range of services ranging from diagnostic assessment, disclosure and provision of post-diagnostic supports.

[1] Trans-disciplinary teams can share common functions and team members are sufficiently familiar with others roles in the team such that roles can be deliberately blurred in order to benefit patient care (e.g., assessment and care co-ordination)
