Details of the Service

The persons appointed will work as part of a dynamic supportive social work team and within a multi-disciplinary team. He or she will deliver a co-ordinated approach to client care within the Crumlin Community Healthcare Network area. He/she will provide a direct point of access for the local community and offer initial and holistic social work assessment and intervention to individuals, families and groups. There are opportunities for group work in a supportive environment.

The professional reporting relationship for clinical governance and clinical supervision will be to the Principal Social worker through the professional line management structure. The line manager for operational issues will be the Community Healthcare Network Manager.
