DNCC Operational Team Lead Integrated Care Programme Chronic Disease

The Operational Team Lead Integrated Care ICPCD Specialist Community Team will play a key role in implementing the model of care for the prevention and management of chronic disease within the IHA Dublin North City and West and IHA Dublin North. The post holder is responsible for coordination of ICPCD Specialist team and Hub operations, ensuring the provision of a seamless integrated service and multidisciplinary input for patients with chronic disease.

The current vacancy is in Dublin North City and West Integrated Health Area, North East Region.

The post will initially be based in Dublin North Central Integrated Care Centre, St Clares, Glasnevin moving to Ballymun Primary Care Centre in approximately 18 months
