Responsibilities & Accountabilities:
- To provide a direct Medical Social Work service to patients, their partners and families as assigned by the Principal Medical Social Worker
- To keep the Principal Medical Social Worker informed on matters pertaining to casework practice particularly in relation to: Critical cases, Critical Department/Hospital Matters, Medical Social work team developments;
- To provide counselling, information and support as appropriate to hospital patients as per assigned caseload;
- To keep and maintain such records and statistics as are part of the structure of the Medical Social Work Department;
- To provide case management support for complex discharge planning;
