Purpose of the Post

In the context of the person-centered philosophy of the Community Nursing Unit and the developing primary care teams and health and social care networks, the post holder will support service users to continue to exercise independence and autonomy in leading a full life no matter what their level of dependency. In conjunction with the multidisciplinary team s/he will assist with the provision of health and social care. This will include all activities of daily living in line with the needs and wishes of the individual and as outlined in an agreed care plan.

Duties assigned will vary depending on the care setting.

 Principal Duties and Responsibilities

· Support clients / residents in directing their own lives in accordance with their cognitive and physical abilities in an environment that reflects the comforts of home.

· Support improved opportunities for clients / residents to have meaningful relationships with families and friends by making them welcome partners in the Community Nursing Unit or at home.

· Respect clients / residents, their families and each other as individuals showing dignity, courtesy and professionalism at all times

· Act as an advocate for clients / residents

· Actively promote the participation of clients / residents in the running of the unit

· Promote the social interaction of clients / residents with the community, including facilitating the ongoing involvement in their personal interests, hobbies etc.

· Assist the Registered Nurse in the provision of a quality service by promoting and adopting

a philosophy of care and working in line with national and locally devised policies and regulations

· Ensure that care is carried out in an empathetic and ethical manner and that the dignity and spiritual needs of the client / resident are respected

· Contribute to the development of a multidisciplinary assessment and care plan and assist in

its implementation and evaluation in consultation with the client / resident and family / carer as appropriate

· Carry out assigned and delegated responsibilities involving indirect care – domestic or catering duties and all activities of daily living under the supervision of a Registered Nurse, e.g. to assist clients maintain standards of personal hygiene, laundry, dietary intake, physical, mental health and any other personal needs.

• Encourage and promote recovery philosophy with service users

• Prepare and serve food as appropriate.

• Attend staff meetings and contribute constructively to the smooth running of the unit.

• Contribute to the maintenance of updating of client/ resident documentation.

• Maintain the confidentially of all information made available to him/ her during the course of his / her work

• To be responsible for the routine cleaning of allocated area of responsibility in accordance with the HSE Policies.

• To operate cleaning machinery safely and ensure that it is kept clean, maintained and appropriately stored.

• To ensure cleaning schedules are adhered to and signed off on as directed by supervisor.

• To ensure that all equipment and materials are appropriately stored

Health & Safety

·        Conduct his/herself in a manner that conveys respect of the individual and ensures safe client/resident care.

·        Report any incident or potential incident which may compromise the Health and Safety of clients. residents, staff or visitors and take appropriate action.

·        Ensure completion of incident/near miss forms.

·        Not undertake any duty related to client/resident care for which he/she is not trained.

·        Understand and adhere to all HSE policies, guidelines and procedures.

·        Apply precautions at all times to minimize the risks of cross infection e.g clean up any spillages of bodily fluids.

·        Ensure that all equipment is stored and cleaned correctly, and storage areas are kept clean and tidy.

·        Report any breakage or malfunction of equipment to technical staff and /or appropriate others.

·        Present to work wearing the agreed attire and identification, having regard to the highest standard of attire and personal hygiene.

Education & Training:

·        Attend induction and mandatory in-service education

·        Maintain continuous personal development and participate in team based development, education and online training and learning on HSELand website.

·        Develop and use reflective practice techniques as part of the multidisciplinary team.

·        Participate in appraisal and the development of a personal development plan in conjunction with his/her line manager.

Administrative

·        Maintain and monitor appropriate levels of consumables supplies in the unit.

·        Maintain appropriate records to facilitate the ordering process/enable cost monitoring in line with financial regulations.

·        Answer the telephone, respond to queries, take messages and deal with queries as appropriate / agreed with the Registered Nurse.

·        Promote a culture that values diversity and respect in the workplace.

·        Record all interventions used under the guidance of the registered nurse.

Eligibility Criteria

Qualifications and/or experience

Candidates must, on the latest date for receiving completed application forms possess:

. (a) Professional Qualifications, Experience, etc

  • Possess the relevant QQI Further Education and Training (FET) Level 5 Certificate in Health Service Skills
  • OR

  • FETEC Level 5 Certificate in Health Service Skills or Healthcare Support.
  • OR

  • A relevant healthcare qualification.
  • OR

  • Be currently employed as an attendant, multi task or a comparable role and be willing to undertake a QQI/FET Level 5 programme in HEALTH Service skills or equivalent.
  • AND

    (b) Candidates must have the personal competence and capacity to properly discharge the duties of the role.

    A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.

    Tenure

    The current vacancies are permanent/temporary and whole time/part time.

    The post is pensionable. A supplementary panel may be created from which permanent and specified purpose vacancies of full or part time duration may be filled in CHO 1 - Donegal. The tenure of these posts will be indicated at “expression of interest” stage.

    Appointment as an employee of the Health Service Executive is governed by the Health Act 2004 and the Public Service Management (Recruitment and Appointments) Act 2004 and Public Service Management (Recruitment and Appointments) Amendment Act 2013.

    Remuneration

    The salary scale for this post as of 01/10/2023 is:

    32,473 33,908 35,420 35,822 36,809 37,662 38,892 40,166 41,488

    New appointees to any grade start at the minimum point of the scale. Incremental credit is normally granted on appointment, in respect of previous experience in the Civil Service, local authorities, health service and other public service bodies and statutory agencies.

    Working Week

    The standard working week applying to the post is 39 hours.

    HSE Circular 003-2009 “Matching Working Patterns to Service Needs (Extended Working Day / Week Arrangements); Framework for Implementation of Clause 30.4 of Towards 2016” applies. Under the terms of this circular, all new entrants and staff appointed to promotional posts from Dec 16th 2008 will be required to work agreed roster / on call arrangements as advised by their line manager. Contracted hours of work are liable to change between the hours of 8am-8pm over seven days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement (Implementation of Clause 30.4 of Towards 2016).
