Physiotherapist Medical Wards Rehabilitation Senior

The successful candidate will be based on general medical wards with the goal to optimise patients’ health and function while also aiding patient flow through the understanding and development of patient pathways.
This caseload may include but is not limited to patients with generalised decreased function, neurological conditions, older persons and frailty & cardiorespiratory conditions.
The purpose of the role is:
- To provide a holistic, multidisciplinary approach to the care of individuals with acute and chronic medical conditions
- To perform multi system assessments e.g. mobility and cardiorespiratory, to help restore and maintain optimum function of patients on medical wards.
- To provide clinical and professional supervision to junior staff while ensuring optimum organisational performance of the service.
- To be responsible for the provision of a high quality Physiotherapy service in accordance with standards of professional practice
- To work in conjunction with other team members in co-ordinating and developing the service to meet the needs of the population it serves in line with the objectives of the organisation
- To work with the Physiotherapist Manager or designated person in ensuring the co-ordination, development and delivery of a quality, client centred physiotherapy service across and between networks in the geographical area.
- To carry out clinical and educational duties as required
- Devise and develop appropriate programs of care for the patient’s journey based on best practice
- To develop innovative service delivery models using telehealth facilities
- To work closely with the older persons and cardiorespiratory teams across the hospital providing cover as required across wards and emergency department.
