Job Title: Rapid Active Support Project (RASP) Support Worker

Location: Belfast (Staff may be required to work outside their direct local area
dependent on business needs).

The Reablement and Support Project (RASP) is all about restoring maximum independence within 2-6 weeks (or longer if needed). As a RASP Support Worker, you will be responsible for providing high-quality, person-centred care to individuals who require immediate assistance with the goal of helping them regain independence. With tailored one-to-one or two-to-one support, this service operates 7 days a week to make a lasting difference in the lives of those you support.

You will work directly with individuals, including those with learning disabilities, mental health issues, and other complex needs, to help them achieve their personal goals, improve their skills, and promote self-sufficiency.

Key Responsibilities:
Homecoming Preparation: Shop for essentials, collect prescriptions, and ensure a comfortable home environment for the individual’s return.
Encouraging Independence:
Support daily living tasks like household chores, shopping, laundry, and meal prep to restore confidence and skills.
Promoting Mobility: Assist with transfers, mobility indoors and outdoors, and help individuals navigate their home.
Personal Care Support: Help individuals regain confidence in managing their personal care independently.
Active Support Planning:
Collaborate on Person-Centred Active Support Plans, set achievable goals, and monitor progress.
Medication Assistance: Support individuals with prescribed medications in line with policy.
Family Engagement:
Communicate with carers and family to involve them in the process of restoring independence.
Adapt to Needs:
Track changes in individual needs and update the Project Manager.
Record Keeping: Maintain accurate records, including Active Support Plans and Progress Reports.

Flexible Work: Work on a rota basis (7 AM - 11 PM, including weekends) to deliver person-centred care.
Training & Supervision: Participate in induction, training, and regular supervision.
Other Duties: Be flexible to take on additional tasks as required.

Terms and Conditions

Contract: Permanent
Salary: £13.80 per hour
Hours: Monday-Sunday
Variable contracted hours available (up to 11pm)
Holidays: 25 days + 3 bank holidays per year
(Pro rata for part-time staff)
Probationary Period: 6 months
Notice Period: 4 weeks
