The role of a Regional Transition & Pathways Manager (SCM2)

The Regional Transition and Pathways Manageris a management role responsible for providing strategic leadership, operational oversight and governance in relation to transition planning and pathway development across Corlann South East.

As a Social Care Manager Grade 2 (SCM2), the post holder is required to maintain professional registration with CORU and practise in accordance with the Social Care Workers Registration Board Code of Professional Conduct and Ethics. The post holder will provide professional leadership and ensure that practice is informed by relevant legislation, regulatory requirements and recognised standards of social care practice.

The post holder will provide leadership and oversight of regional transition planning processes and will be accountable for ensuring that service recommendations, pathway proposals and future service requirements are identified, developed and presented to the Regional Services Management Team to support informed decision-making, service development and resource planning.

The role has overall responsibility for the planning, coordination and governance of school leaver transitions and adult service pathways, ensuring that individuals moving from school and children's services into Corlann adult services experience seamless, person-centred, rights-based and outcome-focused transitions.

The successful candidate must:

· Be registered in the Social Care Workers Register maintained by the Social Care Workers Registration Board maintained by CORU

Or

Be eligible under the Section 91 (existing practitioner) route for registration in the Social Care Workers Register maintained by the Social Care Workers Registration Board maintained by CORU (evidence of application status is necessary)

· Hold a full clean driving licence and access to a vehicle.

Essential Experience

·Minimum of five years' post-qualification experience in disability services, including significant leadership, supervisory or management responsibilities.

• Competitive Rates of Pay (€62,497 - €71,513) pro rata
