Registered Advanced Nurse Practitioner, RANP Older Persons
The role will contribute to innovative models of care, including ambulatory pathways, virtual care and hospital avoidance initiatives, aligned with HSE policy, Slaintecare, Enhanced Community Care and the National Clinical Programmes.
As an autonomous and expert advanced practitioner, the RANP will provide safe, timely and evidence-based nurse-led care, undertaking comprehensive assessment, diagnosis, treatment, care planning and discharge within agreed policies, protocols and guidelines.
The RANP will demonstrate advanced clinical knowledge, critical thinking, complex decision-making and clinical leadership, while contributing to professional scholarship, education, research and the development of evidence-based practice and policy.
The RANP will also contribute to the ongoing development of the ICPOP service in Carlow/Kilkenny and inpatient rehabilitation pathways for older adults, in conjunction with the ICPOP Consultant Geriatrician.
