Service Manager, Older Person Services
The position of Service Manager for Older Persons has responsibility for Sligo/ Leitrim/ West Cavan Home Support Services, Community hospitals and nursing units, Day Services and Community & Voluntary third party Services to include Service Level Agreements and their Governance. The service Manager is responsible for a wide range of services to older people in Sligo Leitrim & West Cavan in partnership with the individual, families, Acute hospitals, Mental Health Services, G.P.’s, Allied Healthcare Professionals and Community Services.
The Service Manager is responsible for the management of Home Care Services across Sligo Leitrim & West Cavan.
There are 1520 people in receipt of Home Support Services with approximately 60,365 hours of care delivered in the home each month through direct and indirect provision.
The Service Manager is the Provider Representative for HSE in respect of the Registration with the Health Information and Quality Authority (HIQA). All 5 community hospitals and nursing units are subject to registration and regulation with HIQA.
There is a total bed capacity of 227 beds across 5 HSE Community hospitals and nursing units. 195 provide long term care, 32 beds provide; Rehabilitation, Assessment, Respite, and dementia respite and Palliative Care. Contracted residential placements are provided in collaboration with private providers through the NHSS and Section 39 service arrangements St Johns Community Hospital provide day Hospital services.
Social Day Care Services are provided to support Older People to remain in their home and local community for as long as possible. There are 4 Social day care services, provided directly by the HSE and governed by the Service Manager. There are also an additional 5 Social Day Centres in Sligo with a further 5 Social Day Centres in Leitrim and one in West Cavan governed through service level and grant aid agreements with various voluntary agencies. There are also 2 dementia specific day services 1 in Sligo and 1 in Leitrim governed by Alzheimer Society of Ireland. There are 57 Active Age Groups funded by the HSE and managed by Leitrim Development Company and Sligo Leader governed through a Service Level. These services provide support which maintains older people in their home and local community for as long as possible.
The Sláintecare Report (2017) and Sláintecare Implementation Strategy (2018) signals the direction for the delivery of health and social care services in Ireland with the potential to create a far more sustainable, equitable, cost effective system and one that delivers better value for patients and service users. At its core, the strategy focuses on establishing programmes of work to move to a community-led model, providing local populations with access to a comprehensive range of non-acute services at every stage of their lives. This will enable our healthcare system to provide care closer to home for patients and service users, to be more responsive to needs and deliver better outcomes, with a strong focus on prevention and population health improvement.
Sláintecare positions Community Healthcare Networks (CHNs) as the ‘fundamental unit of organisation for the delivery of services’ in the community. CHNs are geographically-based units delivering services to an average population of 50,000. The implementation of CHNs will see a coordinated multi-disciplinary approach to care provision, providing better outcomes for people requiring services and supports both within and across networks.
The development of CHNs is a critical step in transforming our healthcare system and will enable real change that will be experienced by all who use our services and work in the HSE.
The Delivery Plans are prepared consistent with related national policies, frameworks, performance targets, standards & resources. It sets out the type and volume of services which will be provided directly or through a range of agencies funded by us, and the actions which we will take to deliver on the goals of the HSE Corporate Plan 2025 - 2027 over the course of the year.
