A panel may be created for Clinical Specialist Physiotherapist in Urology (Pelvic Health) from which permanent and specified purpose vacancies of full or part time duration may be filled.

Details of Service

This role has been created as part of the development of the Continence pathway. The Continence pathway is one of a number of modernised care pathways (MCPs) developed by the National Clinical Programme in Surgery (NCPS) and the HSE as a core component of the transformation of the delivery of scheduled care services.

Sláintecare

The Sláintecare Report (2017) and Sláintecare Implementation Strategy (2018) signal a new direction for the delivery of health and social care services in Ireland with the potential to create a far more sustainable, equitable, cost effective system and one that delivers better value for patients and service users. At its core, the strategy focuses on establishing programmes of work to move to a community-led model, providing local populations with access to a comprehensive range of non-acute services at every stage of their lives. This will enable our healthcare system to provide care closer to home for patients and service users, to be more responsive to needs and deliver better outcomes, with a strong focus on prevention and population health improvement.

Internationally, the strategic repositioning of health services is recognised as a better approach to meet the challenges of escalating demand from an ageing population and the prevalence of chronic diseases, while at the same time ensuring better access to care, addressing inequalities in health and delivering sustainability and best value for population health.

The enhanced community care model is currently being implemented with specialist teams for management of chronic disease and care of the older persons and public health nursing will play a significant role in this change model.

Urology Model of Care for Ireland (2019) envisages a system of urology care that serves the majority of patients, in the primary care setting, while assuring appropriate clinical governance to support a safe and high quality service. Its implementation in full is necessary for an efficient and economically viable service so that we can improve the access of patients across Ireland to the services they require as they get older.

This includes preventative and curative interventions encompassing clinical work, care planning, assessments, reviews, liaison with relevant statutory and voluntary services, and work in both clinical and domiciliary settings.

Clinical Specialist Physiotherapists in Urology (Pelvic Health) have specific responsibilities in the area of clinical care, support, training, etc., in the area of urinary and bowel dysfunction. This work with relevant patient cohorts is essential in optimising physical, social and mental health and well-being of persons with such clinical needs and is an integral post in the Physiotherapy Department in provision of holistic care to eligible patients across the life continuum.

Reporting Relationship

The Clinical Specialist Physiotherapist, Urology (Pelvic Health) will report to

· The appropriate Physiotherapy Manager in Sligo/Leitrim/South Donegal/West Cavan physiotherapy for clinical governance, attendance and service performance management.

Purpose of the Post

  • The aim of these posts is that those referred to the Urology Continence service will receive the right care, at the right time, in the right place in keeping with Slaintecare principles.
  • To provide a first point of contact community Urology Service, encompassing the physiotherapy Advanced Assessment, management and treatment of patients experiencing urinary or bowel incontinence, in conjunction with other member of the MDT, to provide a continuum of care.
  • This post will facilitate specialist interdisciplinary assessment in order to reduce waiting times and improve care delivered to patients within the Urology Continence service.
  • To work with Urology Consultants, Continence Nurse Advisors and other relevant health care professionals in undertaking the triage of ambulatory urology related referrals (efficient and effective management of the waiting lists) to improve the pathway of care within this group, thereby enhancing efficient access to the related services as required.
  • To act as a lead in the development of the Physiotherapy service provided to clients under the National Urology Model of Care 2019. Taking a lead in implementing the improved service model for Urology across primary care in CHO West and as an in-reach secondary care service within the Saolta hospital group.
  • The post holders will be required to collect and analyse data to evaluate effectiveness of their input. They will be required to produce and present reports intermittently as required.
