Location of Post
There is currently one Senior Pelvic Health/Primary Care post within the Sligo/South Donegal CHN to be filled. The post will be in Carrick on Shannon 1 day per week and Sligo/South Donegal CHN base (TBC) the remainder.
A panel may be created for Senior Physiotherapist Pelvic Health/Primary Care
from which permanent and specified purpose vacancies of full or part time duration may be filled.
The successful candidate may be required to work in any service area within the vicinity as the need arises.

Sláintecare
The Sláintecare Report (2017) and Sláintecare Implementation Strategy (2018) signal a new direction for the delivery of health and social care services in Ireland with the potential to create a far more sustainable, equitable, cost effective system and one that delivers better value for patients and service users. At its core, the strategy focuses on establishing programmes of work to move to a community-led model, providing local populations with access to a comprehensive range of non-acute services at every stage of their lives. This will enable our healthcare system to provide care closer to home for patients and service users, to be more responsive to needs and deliver better outcomes, with a strong focus on prevention and population health improvement.
Internationally, the strategic repositioning of health services is recognised as a better approach to meet the challenges of escalating demand from an ageing population and the prevalence of chronic diseases, while at the same time ensuring better access to care, addressing inequalities in health and delivering sustainability and best value for population health.
The enhanced community care model is currently being implemented with specialist teams for management of chronic disease and care of the older persons and public health nursing will play a significant role in this change model.
Senior Physiotherapists in Primary Care/Pelvic Health have specific responsibilities in the area of clinical care, support, training, etc., in Primary Care/Pelvic Health. This work with relevant patient cohorts is essential in optimising physical, social and mental health and well-being of persons with such clinical needs and is an integral post in the CHN in provision of holistic care to eligible patients across the life continuum.
Reporting Relationship
The post holder will report to the Physiotherapy manager for clinical governance and to the relevant network manager/operational lead for operational governance.
Purpose of the Post
· To work in conjunction with other team members in co-ordinating and developing the Primary Care/Pelvic Health service to meet the needs of the population it serves, in line with the objectives of the organisation.
· To be responsible for the provision of a high quality physiotherapy service in accordance with standards of professional practice.
· To carry out clinical and educational duties as required
· To work with the Physiotherapy Manager and CHN team in ensuring the co-ordination, development and delivery of a quality, client centred physiotherapy service across and between networks in the geographical area.
· To deliver Primary Care/Pelvic Health services in line with best clinical practice.
· To develop and implement strategies as part of the Primary Care/Pelvic Health service for delivering effective physiotherapy care within a changing environment.
