Details of service
Sláintecare positions Community Healthcare Networks (CHNs) as the ‘fundamental unit of organisation for the delivery of services’ in the community. CHNs are geographically-based units delivering services to an average population of 50,000. The implementation of CHNs will see a coordinated multi-disciplinary approach to care provision, providing better outcomes for people requiring services and supports both within and across networks.
The development of CHNs is a critical step in transforming our healthcare system and will enable real change that will be experienced by all who use our services and work in the HSE.
The Delivery Plans are prepared consistent with related national policies, frameworks, performance targets, standards & resources. It sets out the type and volume of services which will be provided directly or through a range of agencies funded by us, and the actions which we will take to deliver on the goals of the HSE Corporate Plan 2025 - 2027 over the course of the year. The position of Manager for Home support, Older Persons community and Voluntary services and day services will have responsibility within CHA Sligo South Donegal and / Leitrim/ West Cavan/
This Manager will be responsible for home support to the population living in Sligo Leitrim & West Cavan/ South Donegal in partnership with the individual, families, Acute hospitals, Mental Health Services, Disability Services, G.P.’s, Allied Healthcare Professionals, PHN and Community Services, and older persons community and voluntary and day services.
The Sláintecare Report (2017) and Sláintecare Implementation Strategy (2018) signal the direction for the delivery of health and social care services in Ireland with the potential to create a far more sustainable, equitable, cost-effective system and one that delivers better value for patients and service users. At its core, the strategy focuses on establishing programmes of work to move to a community-led model, providing local populations with access to a comprehensive range of non-acute services at every stage of their lives. This will enable our healthcare system to provide care closer to home for patients and service users, to be more responsive to needs and deliver better outcomes, with a strong focus on prevention and population health improvement.
Reporting Relationship and key working relationships
The post holder will operationally report to both Community Healthcare Area General Managers and line manager support will be appointed to one GM.
Key working relationships
The post-holder will work closely with all members of Community Care Teams, Senior Management Team, Heads of Discipline and Service Managers within SLSDWCIHA The post-holder will also liaise with colleagues external to the HSE e.g., funded agencies, statutory bodies, hospital groups, universities etc. Key working relationships associated with the role include:

· Community Healthcare Network Manager and teams
· Bed Management and Patient Flow leads in acute and community.

· Inter-rai Educator and facilitators

· Quality Safety and Risk Management team, to include Health and Safety and risk management teams. Estates and Maintenance Departments.

Purpose of the Post
The post of Manager for Home Support , Community and voluntary Services is a key role involving overall responsibility for the delivery of home support services to the population. The role requires strong governance and leadership within the statutory services and requires close working partnerships with the Voluntary and Private Care sectors.
The Manager will have operational responsibility for:
· Home Support
· Community and Voluntary and Day services including meals on wheels.
· key contact for governance and management of service level agreements and arrangements including performance monitoring.
· line management responsibility for operational manager for home support; Home Support Resource Managers and all administrative staff (directly employed) within the above functions.
In additional the post holder will:
· Ensure the efficient, effective and safe delivery of health and personal social services for patients and clients, within HIQA regulations, national, and IHA frameworks and available resources ·
· Ensure that appropriate standards are maintained with regard to ethics, confidentiality and legislation. ·
· Adequately identify, assess, manage and monitor risk within their area of responsibility.
· Provide leadership to staff in delivering services consistent with the new model of care to ensure integrated service for patients and clients
· Support the delivery and governance of integrated service provision, ensuring safety and quality of service
· Create a management team which supports collective responsibility and action in all aspects of Health and Personal Social Service delivery
