Autism transition pathway project / Laois Day Services
Permanent Part-Time (60 hours per fortnight)
Monday to Friday Day Service
We are looking for an ambitious and enthusiastic Social Care Worker to work as part of an exciting new transition pathway project for adults with autism transitioning from school to adult day services. This role will involve working closely with the individuals transitioning to day services, the Transition Pathway Project Manager, and the multi-disciplinary team. The role will require the Social Care Worker to engage with adults to support them through the transition process from school to adult day services.
This may include developing trusting relationships with the individuals, supporting them through the transition process from finishing school and through the transition to develop the exciting world of opportunities that day services can offer. The Social Care Worker will be supported by the Project Manager to build a strengths based and autism informed approach to supporting individuals with the transition to this new and exciting period of life post school.
Once the individuals have successful transitioned today services the Social Care Worker will work as part of a dynamic team supporting Individuals within the Laois Day Services. The successful candidates will be provided with the opportunity to support individuals with independent living skills, community inclusion and the promotion of their personal health and wellbeing. They will be given the opportunities and resources to encourage and motivate individuals in pursuing their personal interests and life goals.
Requirements:
- Relevant recognised Diploma in Social Care / Applied Social Studies in Social Care (Level 7 on National Framework of Qualifications) Please note social care candidates must be registered with or eligible to register with CORU by November 2025.
- Experience of supporting adults with autism
- Experience supporting adults who may engage in behaviours of concern
- Experience of delivering quality supports with achievable outcomes
- Knowledge of New Directions Interim Standards and Person Centred Plans
- Excellent organisational, logistical and I.T skills an advantage
- Lone working/Individual Support
- Full Current Driving Licence
- Suitability as outlined in the introduction to the role.
- We require staff members to be willing to work alone in Day Services.
