Children’s Disability Network Teams (CDNTs)
Grade IV (Clerical) | Fixed Term 12 Months | Full Time (35 hours per week)

The Central Remedial Clinic (CRC) is seeking an enthusiastic and compassionate Social Prescribing Link Worker to join our Children’s Disability Network Teams (CDNT's) in North Dublin. (Main location: Swords CDNT).

This exciting role supports CRC’s commitment to delivering person-centred, holistic services for children with disabilities and their families. As a Social Prescribing Link Worker, you will act as a vital bridge between clinical services and community-based supports, helping families access local resources, activities, organisations and services that enhance wellbeing, inclusion and participation.

About the Role

Working closely with families, clinicians, community organisations and statutory services, you will:

• Conduct holistic assessments and develop strengths-based wellbeing plans with families.
• Support children and families to access community, voluntary and social supports.
• Provide one-to-one and group-based support using a personalised coaching and co-production approach.
• Advocate for families and assist them in navigating community services and statutory supports.
• Build and maintain a comprehensive directory of inclusive community resources.
• Develop strong partnerships with community and healthcare stakeholders.
• Monitor outcomes and contribute to service evaluation, reporting and quality improvement initiatives.
• Ensure compliance with safeguarding, GDPR, health and safety and organisational policies.

About You

The successful candidate will have:

Essential Requirements

• An appropriate third-level qualification in a Social, Community, Health or related discipline.
• A minimum of two years’ experience in community development, healthcare or a related field.
• Experience supporting individuals and families in one-to-one and/or group settings using a strengths-based approach.
• Strong interpersonal skills with the ability to build trusting relationships.
• Excellent organisational, communication and IT skills.
• Experience in data collection, monitoring and reporting.
• An understanding of the wider social determinants of health.
• A full driving licence valid in Ireland and access to own transport for community-based work.

Department of Health Salary Scale applies.Grade IV (Clerical), Grade Code 0558. Salary Scale: €36,470 – €56,018 (inclusive of 2 LSIs).
