Support Coordinator
Working Hours: 35 Hours per week 5 over 7 days.
Salary: €35,000
Support Coordinators play a critical role in facilitating practical support and coordinating services for older people to address their challenges effectively. The role focuses on assessing needs, connecting with partners, and coordinating services to ensure a comprehensive and targeted approach.
Responsibilities
Aligned with ALONE’s mission and values, this role focuses on coordinating services to ensure older people receive the supports they need, without directly delivering frontline care.
- Develop personalised support plans, connecting older people to ALONE’s services and external community supports to enhance their quality of life.
- Streamline access to health, social care, and welfare services, advocating for older people's needs to improve service delivery.
- Build and maintain strong referral networks with statutory and non-statutory agencies, ensuring efficient service coordination.
- Maintain accurate records in ALONE’s CRM to support service planning and delivery.
- Adhere to ALONE’s quality management policies, monitor key performance indicators (KPIs)
- Oversee the distribution of assistive technology
Essential skills & experience
- Relevant experience in social care, social work, or community development, with 2+ years of experience in a similar role.
- Experience in service coordination. Developing personalised support plans and advocating for older people’s rights is an advantage.
- Strong knowledge of health, social care, and welfare systems to improve service access.
- Excellent interpersonal and communication skills to engage effectively with older people, families, and stakeholders.
- Ability to build and maintain referral networks with statutory and non-statutory agencies.
- A collaborative team player who fosters strong internal and external relationships.
- Proactive problem-solver, using innovation to improve service delivery and support independent living.
- Strong time management and organisational skills to prioritise workload and meet KPIs.
- Proficiency in CRM systems and digital tools for data management and service planning.
- Experience in partnership development, quality management, and performance monitoring in a service setting.
- A clean full drivers licence with your own car is essential
