Overview of the Post: To work as part of a dedicated and innovative team in a community based house providing respite and holiday breaks for adults with physical and/or sensory or mild to moderate intellectual disabilities, within a quality framework. The post holder will be responsible for providing excellent professional, skilled and effective person centred, evidence based nursing care. Overview of Duties & Responsibilities: · Deliver a high standard of nursing care to service users within a best practice/evidence based framework. · Day to day management of designated caseload including planning, assessment and intervention using researched based practice. · Be visible and accessible to service users, families and carers and work in partnership with them to ensure efficient, effective and caring services. · Develop collaborative, needs based care plans, through assessment, planning, implementation and evaluation. · Ability to assess plan and respond to the nursing needs of service users with epilepsy, respiratory difficulties and other relevant presenting conditions. · To comply with legal requirements for the safe storage and administration of medicines and other clinical products. · To promote and deliver services within the ethos and philosophy of Enable Ireland. · To promote the health, welfare and social wellbeing of service users in the service. · Be aware of, and comply with, the principles of clinical governance including quality, risk, and health and safety. · Recognise potential risk and risk situations and act accordingly in conjunction with Enable Ireland policies. · To establish and maintain good working relationships with colleagues, families and other key stakeholders. · To organise and participate in clinics held by the rehabilitation consultant. · To establish and maintain appropriate working relationships with other organisations, voluntary agencies etc. · Participation in regular team meetings and other meetings as appropriate. · Undertake the key worker role as appropriate. · Maintain awareness of professional literature, current developments in nursing through self-education, training and professional development. · Participate in clinical and line management supervision and performance management. · Maintaining appropriate, accurate and up to date written records and reports regarding care of service users, in accordance with professional guidelines and Enable Ireland information management policy. · To maintain a high standard of professional behaviour and be accountable for practice. · To work within the scope of practice and take measures to develop and maintain the competence necessary for professional practice. · To work collaboratively with the Adult Services Manager and CNMII. · Development of and participation in training programmes for service user’s colleagues and carers/families, where required. · To carry out vaccination of both service owners/users and staff as required. · Perform any other duties appropriate to the position from time to time. The successful candidate will have: Essential Criteria: · Be registered, or be eligible for registration, in the General Nurse Division of the Register of Nurses kept by the Nursing & Midwifery Board of Ireland [NMBI] (Bord Altranais agus Cnáimhseachais na hÉireann). · Candidates must possess the requisite knowledge and ability, (including a high standard of clinical and administrative capacity), for the proper discharge of the duties of the office. · Eligible to work in the state. Annual registration · On appointment, practitioners must maintain live annual registration on the General Nurse Division of the Register of Nurses & Midwives maintained by Nursing and Midwifery Board of Ireland [NMBI] (Bord Altranais agus Cnáimhseachais na hÉireann). Overview of Duties & Responsibilities: · Provide leadership and support to colleagues around person centred planning and approaches in line with policies. · Assist service users to develop and maintain a “Circle of Support” with relevant participants of the service user’s choice. · To work closely with Keyworkers to facilitate the Service Owner to identify, plan and implement their goals, and support them to actively participate in the activities of their choice. · To follow the Service Owners’ bespoke care plan to ensure their independence, & wellbeing. · To assist the Service Owner with all activities, including therapy programmes in line with their personal goals. · Be aware of health issues for each Service Owner and administer medication accordingly to Enable Irelands medication policy. · Maintain high standards in practice at all times in accordance with Enable Ireland policy. · Liaise with other staff involved with Service Owners as required, for example therapy staff, etc. · Be familiar with the correct usage of Service Owners’ equipment. · Carry out duties in accordance with Enable Ireland Health & Safety procedures and ensuring Service Owners safety is paramount at all times. · Act as a strong team member, sharing information and ideas with co-workers.
