The team has a large geographical spread serving all of Kildare and West Wicklow and is based in St Brigid’s Hospice, The Curragh, Co. Kildare. The palliative home care service is a five day service with weekend service for planned essential calls in the patient’s place of residence. This may be the patients home, community hospital or private nursing home. In the community, the specialist palliative care team work closely with the General Practitioner, Public Health Nurse, community hospitals and nursing homes. Future developments for the service are in line with the national service plan 2025. Community Health aims to deliver an integrated healthcare system which is responsive to people’s needs at the lowest level of complexity for the local population. Focusing on service delivery and decision making at local level informed by national frameworks, allows the HSE to: · provide better direct accountability; · provide for increased decision making at · local area level; and · Deliver services in the community through an integrated management structure.
