Details of Service The Home Care Team in St Brigid’s Hospice provide a valued palliative care service to people in the community in their home.The team structure is composed of clinical nurse specialists who provide care to the person in the home and is clinically led by the palliative care consultant. The team has a large geographical spread serving all of Kildare and West Wicklow and is based in St Brigid’s Hospice, The Curragh, Co. Kildare. The palliative home care service is a five day service with weekend service for planned essential calls in the patient’s place of residence. This may be the patients home, community hospital or private nursing home. In the community, the specialist palliative care team work closely with the General Practitioner, Public Health Nurse, community hospitals and nursing homes. Future developments for the service are in line with the national service plan 2025. Community Health aims to deliver an integrated healthcare system which is responsive to people’s needs at the lowest level of complexity for the local population. Focusing on service delivery and decision making at local level informed by national frameworks, allows the HSE to: · provide better direct accountability; · provide for increased decision making at · local area level; and · Deliver services in the community through an integrated management structure. GENERAL INFORMATION Sligo County Council is the Authority responsible for Local Government in County Sligo. The corporate headquarters are located at County Hall, Riverside, Sligo, and there are three Municipal Districts [Borough District of Sligo (Sligo/Strandhill), Municipal District of Sligo (Sligo/Drumcliffe) and Municipal District of Ballymote-Tubbercurry]. Sligo County Council has 18 elected members and approximately 460 staff. The Chief Executive of Sligo County Council is Mr. Martin Lydon and the Cathaoirleach is Cllr. Dónal Gilroy. Sligo County Council provides a diverse range of services across a large geographic area. Key services areas include Planning, Local Enterprise Office, Community and Economic Development, Transportation, Motor Taxation, Water, Environment, Emergency Services along with Housing, Libraries and the Arts. These operations are supported by internal services which include ICT, Corporate, Finance and Human Resource functions. THE COMPETITION Sligo County Council is currently inviting applications from suitably qualified persons for the post of Administrative Officer (Grade VII). Sligo County Council will, following the interview process, form three panels for the post from which future relevant vacancies may be filled, subject to sanction approval from the Department of Housing, Local Government and Heritage. Speech and Language Therapist, Senior CAMHS 0.8 WTE The speech & language therapist is a core team member in the CAMHS MDT. The CAMHS Speech & language therapist delivers intervention within an integrated CAMHS team. The post holder is responsible for the assessment, diagnosis and management of own caseload of children and young people, presenting with communication issues in the context of moderate-severe mental health needs. The CAMHS SLT participates in differential diagnostic and joint formulation with CAMHS MDT colleagues to ensure timely and accurate intervention. The wider MDT mental health interventions are supported by the CAMHS SLT who aims to enhance clinical outcomes by adapting and co-designing mental health interventions, thereby improving access to, and efficacy of these therapeutic interventions. The person appointed to this post will work as a Senior Speech & Language Therapist in their designated clinical area, taking a lead role in the delivery of a high quality speech & language therapy service. The speech & language therapy team consists of the speech & language therapy manager based in Primary Care, one senior an. The SLT manager meets will all senior therapists individually for clinical supervision. There is a peer supervision group for senior SLTs, and the monthly department meeting is another forum where complex case discussion and other issues can be discussed in the wider SLT team context.
