Outpatient physiotherapy services are provided in MSK, Women’s Health, paediatric, respiratory and lymphoedema. Details of Older Persons’ Service The National Integrated Care Programme for Older Persons’ (NICPOP) is leading out on the development of an end to end pathway that describes cohesive primary, secondary and acute care services for older people with a specific focus on those with more complex needs and frailty. The core aim is to develop and implement specialist multidisciplinary geriatric services, with dedicated specialist in-patient wards, providing Comprehensive Geriatric Assessment (CGA), treatment, rehabilitation and appropriate discharge planning. This pathway for older persons supports better outcomes, reduced readmissions rates, reduced rates of long-term care use, reduced functional decline and improved quality of life. The senior post in Older Persons Services will support patient flow and the safe management of older persons in acute care, from the Emergency department and across to inpatient areas. The focus will be on providing early assessment and intervention for older persons and ensuring safe and timely discharge. The post will provide a 7/7 service ensuring that older persons receive the level of service required 7 days per week. The post holder will work with therapy colleagues as part of a roster. The post holder will be primarily based on the MUH site and will be a key member of the older persons’ physiotherapy team delivering safe and effective physiotherapy care. The post holder will work with the inter-professional team within the service. The Older Persons Physiotherapy Team includes Clinical Specialist, Older Persons, Senior Physiotherapist Medical (inpatients), Senior Physiotherapist Stroke (inpatients) Senior Physiotherapist Older Persons, Senior Physiotherapist Frailty at the Front Door (FFD), staff grades and therapy assistants. MWCH Principal Clinical Psychologist
The HSE Mid-West Young Adult Disability Team, aims to improve the experiences of Young People transitioning from child disability services to adult focused disability services in the Mid-West region i.e., Clare, Limerick and North Tipperary. This specialist multidisciplinary team (MDT) for Young People with disabilities is provided by the HSE Mid-West Young Adult Disability Team (Principal Psychologist and Senior Grade Occupational Therapist and Psychology Assistant. The service is delivered in line with the approved Model of Service document developed in 2022. The overall purpose of the post is to put in place and provide clinical leadership and pivotal service for the Young Adults Disability Team. Under the model of service for the Young Adult Disability Team in the Mid-West Community Healthcare the “ Psychologist will also act as Clinical Lead of the Young Adult Disability Team, a role that will require 0.5 WTE allocation” . This level of responsibility for service lead and development are the duties of the Principal Psychologist in addition to clinical responsibilities. Currently young adult service users attending this service, the majority of whom have received a diagnosis of Intellectual disability and Autism, do not have access to other pathways for psychology services despite the established complexity of their needs. This post forms part of the overall resourcing of services within CHO Mid-West. The post covers Mid-West Community Healthcare.
