This service will be delivered in line with the National Clinical Programme model for stroke care, aiming to ensure a seamless transition for patients who are medically fit to return home from the acute hospital setting. To support the stroke patient journey as they transition from inpatient care to community based services, ensuring coordination of care and optimisation of available resources. The post holder will also be responsible for collecting service data, to demonstrate the effectiveness of the ESD model, and will participate in service innovation initiatives in conjunction with other team members. There will be opportunities to contribute to ongoing and new service development projects, as well as research activities on site. Eligibility Criteria 1. Statutory Registration Professional Qualifications, Experience, etc (a) Candidates for appointment must: (i) Be registered on the Occupational Therapists Register maintained by the Occupational Therapists Registration Board at CORU ( https://www.coru.ie ) AND (ii) Have three years full time (or an aggregate of three years) post qualification clinical experience in clinical setting. AND (iii) Have the requisite knowledge and ability (including a high standard of suitability and professional ability) for the proper discharge of the duties of the office. AND (iv) Provide proof of Statutory Registration on the Occupational Therapists Register maintained by the Occupational Therapists Registration Board at CORU before a contract of employment can be issued. 1. Annual registration (i) On appointment, practitioners must maintain annual registration on Occupational Therapists Register maintained by the Occupational Therapists Registration Board at CORU AND (ii) Practitioners must confirm annual registration with CORU to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC) 3.
