To support the stroke patient journey as they transition from inpatient care to community based services, ensuring coordination of care and optimisation of available resources. The post holder will also be responsible for collecting service data, to demonstrate the effectiveness of the ESD model, and will participate in service innovation initiatives in conjunction with other team members. There will be opportunities to contribute to ongoing and new service development projects, as well as research activities on site. Eligibility Criteria 1. Statutory Registration Professional Qualifications, Experience, etc (a) Candidates for appointment must: (i) Be registered on the Occupational Therapists Register maintained by the Occupational Therapists Registration Board at CORU ( https://www.coru.ie ) AND (ii) Have three years full time (or an aggregate of three years) post qualification clinical experience in clinical setting.
