SLMH Early Intervention In Psychosis Key Worker For Demonstration Sites

Organisational Area Community Healthcare Organisation 1 – Sligo /Leitrim /Donegal and West Cavan Location of Post The designated base will be in Sligo Leitrim Mental Health Service, as part of EIP demonstration site. A panel may be created for Early Intervention in Psychosis Key Worker from which future vacancies of full or part time duration may be filled. Details of Service The National Clinical Programme for Early Intervention in Psychosis (EIP) established by the HSE and working in partnership with the College of Psychiatrists of Ireland is developing a Model of Care. It incorporates early detection, specialist assessment of individuals with their first episode of psychosis (FEP) or an at risk mental state (ARMS) followed by engagement with the provision of multidisciplinary evidence based interventions to ensure that people who develop psychosis for the first time are engaged and receive standardised effective treatment in a timely manner. In 2018 the HSE has funded three EIP demonstration sites to deliver services for service users with first episode psychosis (FEP) The HSE Model of Care incorporates evidence from national and international studies over the last twenty years where EIP services have been developed. The MOC applies to everyone from 14 to 65 years of age. It is estimated that 1700 people develop psychosis annually in Ireland although there is limited Irish data available to support this currently. The overall expected incidence is 32 cases per 100,000 population although this can vary considerably depending on demographics, population density, socio-economic factors etc. For individuals referred with a first episode of psychosis it gives clear recommendations for engagement with an EIP key worker, a comprehensive assessment which is led out by the EIP key worker and the Consultant Psychiatrist and the provision of core evidence based interventions to service users and their families/carers for up to three years. The core interventions comprise medication management, CBTp, service user and family psychoeducation, support and interventions, supported employment and physical health and lifestyle interventions The HSE Model of Care is recommending a staged implementation process for this clinical programme commencing with FEP service provision. As part of the National Clinical Programme for Early Intervention in Psychosis the clinical programme established three EIP demonstration sites for the Hub and Spoke model of EIP service provision. The demonstration sites delivered the components of the National Clinical Programme for Early Intervention in Psychosis to all service users with a first episode of psychosis aged (14 -64 years)attending AMHS and CAMHS in the area served by the ‘hub and spoke’ team. The demonstration sites were subject to external evaluation. Reporting Relationship The EIP Key worker post holder is · professionally and operationally accountable to their professional line manager · clinically accountable to the named consultant psychiatrist for named community mental health team · works with the EIP hub team and named consultant clinical leads for the EIP Demonstration site. Purpose of the Post This post of an EIP key worker is critical to the successful roll out of Early Intervention in Psychosis services. Within the National EIP sites the EIP key worker role will focus on First Episode of Psychosis (FEP) service provision. The person will champion and lead on FEP service delivery within their community mental health team based on EIP principles. Key to this role is assertive engagement with service users presenting with a first episode of psychosis, detailed specialist clinical assessment including risk assessment, provision of psychosocial interventions for the service user and their family/carers with follow up for up to three years. Within the MDT led by the Consultant Psychiatrist the EIP key worker will coordinate each service user’s individual care plan and the provision of evidence based interventions by multidisciplinary team members throughout their three year engagement with the clinical programme. A holistic, collaborative, optimistic and recovery approach will underline all of their work with the service user and their family/carers. They will also contribute to the evaluation of the EIP demonstration sites. The key worker will be a member of the EIP hub team and will work to deliver the clinical programme as described for EIP demonstration team.

Recycling Operative
Main purpose of job To be responsible through the Recycling Centre Supervisor to the Area Recycling Officer. The post holder will undertake, usually as part of a small team, duties associated with the operation of a recycling centre. Summary of responsibilities and personal duties 1. To advise, direct and assist members of the public with the segregation and disposal of recyclable materials both non-hazardous and hazardous in nature and in doing so deliver high levels of customer care and promote recycling initiatives. 2. To ensure that only waste types permitted under the waste management licence are accepted on site and that they are stored in an appropriate manner. 3. To take appropriate action, under the direction of the Recycling Centre Supervisor/Area Recycling Officer, where non-permitted waste is detected. 4. To control access and ensure that members of the public are not exposed to risk when using the facilities. 5. To ensure that the site including open areas, buildings and the area immediately outside the site are kept clean and to participate in routine and special cleaning programmes as required, and ensure all spillages and leaks are cleaned and/or contained as a matter of urgency. 6. To assist contractors on site in the delivery of their services, ensure that all contractors comply with the site operational plan and adopt safe systems of work as per risk assessments and report any non-compliance to the Recycling Centre Supervisor/Area Recycling Officer. 7. To inspect and record the quantities and types of all waste removed from the site, to ensure that there is no unauthorized removal of waste and input the necessary data into the council’s IT systems. 8. To file copies of waste transfer documents and other information as required by similar senior management. 9. To record all relevant activities at the site in the official site diary. 10. To manage the use of all waste containers, in the absence of the Recycling Centre Supervisor/Area Recycling Officer and ensure that they are emptied sufficiently frequently. 11. To operate mobile compaction/skip moving equipment and any other plant and equipment provided on site. 12. In the absence of on-site supervision, ensure that safe systems of work are employed and that all work activities are carried out in compliance with health and safety legislation and the waste management site licence. 13. To carry out daily health and safety checks at the site including the inspection of plant and equipment prior to operation, conduct inspection and record maintenance logs and to report any contraventions or defects to the Recycling Centre Supervisor/Area Recycling Officer. 14. To carry out fire safety inspections, maintain site fire log and implement the fire action plan in the event of a fire breaking out in any part of the site. 15. To distribute salt or grit within the site and around the entrance and exit during periods of frost, ice or snow and take any other actions as instructed by the Recycling Centre Supervisor/Area Recycling Officer or above to reduce the likelihood of accident or injury. 16. To take all actions necessary to control dust, odour, noise, pests and litter emanating from the site and to minimize the effects of the site overall. 17. To open, close and work in the recycling centre and civic amenity site at appropriate times as instructed by management and if necessary, in the absence of the Recycling Centre Supervisor/Area Recycling Officer to act as primary key holder for the site and respond to out-of-hours emergencies. 18. To participate in any educational initiatives at the site and liaise as necessary with the enforcement authorities and visitors. 19. To keep up to date with best practice in waste management and to participate in any relevant training programme or initiatives as identified or required. 20. To participate in all induction and in-service training provided by Belfast City Council and in the induction and support of all newly appointed staff and other human resource management policies and procedures, as appropriate, including, absence management, disciplinary and grievance procedure. 21. Job Description - Dispensary Assistant. • Endorse all prescription forms as appropriate. • Label and dispense prescriptions to patients in accordance with the correct operating procedures to ensure safe supply to patients in an efficient and accurate manner. • Issue prepared prescriptions to patients. • Ensure all dispensary equipment is kept clean and in good working order. • All work surfaces are cleaned and tidied on a regular basis.. • Maintain an accurate detailed record of all dispensing transactions. • Promptly forward all invoices and dispensary related correspondence to the necessary department. • Maintain a system for providing and compiling dosettes for those patients requiring a compliance aid. • Support the delivery of medicine use reviews. • Ensure that refrigerated items are stored at the appropriate temperature and maintain the temperature control record/logbook. • Ensure the drugs are received and stored in an appropriate manner in accordance with the manufacturer’s instructions. • Ensure that all medicines and appliances dispensed are checked against the prescription and whenever there is doubt about the appropriateness of the item, or about the dose or labelling instructions to check with the authorising doctor or duty doctor. • Operate efficient stock control appropriate to the needs of the dispensary with the objective of ensuring continuity of supply for patients and minimising wastage. • Undertake other reasonable duties within the framework of the post as directed by supervising manager • Answer telephone inquiries, referring callers to pharmacist when necessary. • Use the nursing home electronic system to a. Add acute medication to the electronic system, understand how to add stock (stock control in doses etc) g. Count and scan prescriptions for month end. h.
