We provide individualised services, centre-based and community-based day and residential service to adults with high support needs, based on personal outcome measures. We strive to provide an excellent service, integrating the community as much as possible into the daily lives of the people supported by the services. The Role: To provide clinical nursing skills in supporting people with intellectual disabilities in achieving optimum level of independence. To exhibit and maintain the highest quality of nursing practice and apply their skills to ensure the physical, social, emotional needs of the people supported by the services in their care are addressed at all times. Be responsible for the provision of nursing care and be an informed resource for educating support workers and family members, which shall benefit people supported by the services. Firstly the Support Worker is responsible for all aspects of personal care required during the day. Reporting to the Strive Project Team Lead, the Community Connector will be an integral component of the transition group team. This team will be focused on delivering key outcomes for individuals specifically in the areas of independent living and personal development. The community connector will work with the service owners to support to enable them to build a lifestyle and live their life as part of their chosen community. In order to facilitate this evening and weekend work will be required and this contract will be over a seven-day roster.
