This restructuring may entail an escalation of grade depending on candidate and site assessments It is noted that the post must be congruent with the requirements of, and facilitate implementation of Sláintecare, and the HSE’s National Framework for the Integrated Prevention and Management of Chronic Respiratory Disease, the National Clinical Programme Respiratory and the Enhanced Community Care Business Case, including commitment to deliver the relevant performance outcomes. .. Purpose of the Post: The CNS Respiratory (COPD Outreach) post holder will deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse/Midwife Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008. Caseload The Clinical Nurse Specialist Respiratory (COPD Outreach) will focus on service users, aged 16 years and older with COPD meeting the inclusion criteria for the COPD outreach service. The concepts are: • Clinical Focus (Direct and Indirect Care) • Service user/client Advocacy • Education and Training • Audit and Research • Consultancy (including leadership in clinical practice) The role of the CNS Respiratory (COPD Outreach) is to provide initial post discharge, person centred care and ongoing management of service users with COPD in the home/normal place of residence. The post holder will work within the overall integrated respiratory service. They will provide physical, psychological and emotional support to an agreed caseload of COPD Outreach service users and their families throughout their COPD trajectory. In order to ensure continuity of service to service users the CNS may be required to rotate other parts of the integrated respiratory service such as Respiratory Integrated Care and the Acute Integrated Care and Pulmonary Rehabilitation. This will also promote service integration and enhance skillset development. It will be dependent on identified service needs at each site.
