The Continence pathway is one of a number of modernised care pathways (MCPs) developed by the National Clinical Programme in Surgery (NCPS) and the HSE as a core component of the transformation of the delivery of scheduled care services. Sláintecare The Sláintecare Report (2017) and Sláintecare Implementation Strategy (2018) signal a new direction for the delivery of health and social care services in Ireland with the potential to create a far more sustainable, equitable, cost effective system and one that delivers better value for patients and service users. At its core, the strategy focuses on establishing programmes of work to move to a community-led model, providing local populations with access to a comprehensive range of non-acute services at every stage of their lives. This will enable our healthcare system to provide care closer to home for patients and service users, to be more responsive to needs and deliver better outcomes, with a strong focus on prevention and population health improvement. Internationally, the strategic repositioning of health services is recognised as a better approach to meet the challenges of escalating demand from an ageing population and the prevalence of chronic diseases, while at the same time ensuring better access to care, addressing inequalities in health and delivering sustainability and best value for population health. The enhanced community care model is currently being implemented with specialist teams for management of chronic disease and care of the older persons and public health nursing will play a significant role in this change model. Urology Model of Care for Ireland (2019) envisages a system of urology care that serves the majority of patients, in the primary care setting, while assuring appropriate clinical governance to support a safe and high quality service. Its implementation in full is necessary for an efficient and economically viable service so that we can improve the access of patients across Ireland to the services they require as they get older. This includes preventative and curative interventions encompassing clinical work, care planning, assessments, reviews, liaison with relevant statutory and voluntary services, and work in both clinical and domiciliary settings. Clinical Specialist Physiotherapists in Urology (Pelvic Health) have specific responsibilities in the area of clinical care, support, training, etc., in the area of urinary and bowel dysfunction. This work with relevant patient cohorts is essential in optimising physical, social and mental health and well-being of persons with such clinical needs and is an integral post in the Physiotherapy Department in provision of holistic care to eligible patients across the life continuum. Advanced Nurse Practitioner Or Candidate
Job Title and Grade: Candidate Advanced Nurse Practitioner (cANP) Endoscopy Bowel Screen or Advanced Nurse Practitioner (ANP) Endoscopy Bowel Screen Grade Code 2267/2272 Location of Post: Sligo University Hospital (Ospidéal Ollscoile Shligigh) Sligo University Hospital (SUH) has a stand-alone Endoscopy Unit. It is located on the fourth floor of SUH and compromises of an admission/discharge/consultation room, 2 patient preparation rooms pre-procedure, 2 procedure rooms, 8 bedded recovery area, step down area post procedures, waiting area and offices. Decontamination facilities for endoscopes are on site within the endoscopy unit. The Endoscopy Unit at SUH is an accredited unit by the UK based Joint Advisory Group (JAG) and also has been an accredited National Bowel Screen Centre since 2012. The unit provides both an inpatient and outpatient service to patients undergoing Endoscopy procedures including colonoscopy, gastroscopy, flexible sigmoidoscopy, bronchoscopy, EBUS, trans oesophageal echo (TOE) and a range of endoscopic interventions. There are currently 8 scheduled GI sessions per week, 2 bronchoscopy sessions and 1 BowelScreen list with the National Screening Service (NSS). There are currently, 2 CNM2’s, a Bowel Screen/ Pre assessment CNM2, Surgical pre assessment CNM2, Triage CNM2, Validation CNM2 and 1 RANP Gastroenterology/BowelScreen (Nurse Endoscopist) The unit has dedicated decontamination personnel. There is currently one permanent whole-time vacancy available for one permanent ANP post or one cANP. If a cANP is appointed they will be expected to progress to ANP and performing colonoscopy procedures independently within two years or who are eligible to register with NMBI in Gastroenterology. The successful candidate may be required to work in any BowelScreen service area within the HSE West North West as the service need arises. A panel may be formed as a result of this campaign for HSE West and North West Health Region from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled.

CHNs are geographically-based units delivering services to an average population of 50,000.
