Clinical governance will be with the GP responsible for the patient in primary care and the relevant Consultant in Cardiology in secondary care. Purpose of the Post: The role of the CNS. CVD will differ according to the needs and configuration of established cardiology services at each site. The purpose of this integrated care CNS CVD post is to: Provide expertise and specialist nursing services to patients with a cardiovascular condition both in hospital outpatient settings and in primary care. The post holder will liaise between acute cardiology services and integrated cardiology services in the community along with other agencies to deliver effective evidenced based care. They will use resources efficiently to achieve the best possible outcomes in keeping with the approved models of care and HIQA standards. The person appointed to this post will work in the newly formed Integrated Care Cardiology Service. The post holder will work as part of a multidisciplinary team (MDT) delivering coordinated evidence based care for patients in primary care whilst liaising closely with secondary care. The Integrated Care CNS CVD will deliver nurse-led clinics to provide support to patients and their GPs in creating management plans, assessing relevant treatments, assisting with diagnosis development and will provide education to patients and staff. This post is 80% working with GP’s in Primary Care, and 20% of CNS CVD role will involve working with the Secondary Care Cardiology Team. In order to ensure continuity of service to patients the CNS CVD may be required to rotate/cover other parts of the integrated cardiology service such as Cardiac Rehabilitation. This will also promote service integration and enhance skillset development. It will be dependent on identified service needs at each site. If deemed appropriate, site rotation can occur within the Cardiology Integrated services within Cavan and Monaghan. The CNS CVD caseload will focus initially on the following patient groups: • Chronic Cardiovascular Disease • Atrial Fibrillations • Ischaemic Heart Disease 1. Statutory Registration, Professional Qualifications, Experience, etc. (a) Eligible applicants will be those who on the closing date for the competition: (i) Be a registered nurse/midwife on the active Register of Nurses and Midwives held by An Bord Altranais and Cnáimhseachais na hÉireann (Nursing and Midwifery Board of Ireland) or be eligible to be so registered. AND (ii) Be registered in the division(s) of the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann) Register for which the application is being made or be entitled to be so registered. OR (iii) In exceptional circumstances, which will be assessed on a case by case basis be registered in another Division of the register of Nurses and Midwives. AND (iv) Have a minimum of 1 years’ post registration full time experience or an aggregate of 1 years’ full time experience in the division of the register in which the application is being made (taking into account (ii) (iii) if relevant) AND (v) Have a minimum of 1 years’ experience or an aggregate of 1 years’ full time experience in specialist area of Cardiovascular Disease Integrated Care. AND (vi) Have successfully completed a post registration programme of study, as certified by the education provider which verifies that the applicant has achieved a Quality and Qualifications Ireland (QQI), National Framework of Qualifications (NFQ) major academic Level 9 or higher award that is relevant to the specialist area of care (equivalent to 60 ECTS or above), and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery 4th ed (2008). Alternatively provide written evidence from the Higher Education Institute that they have achieved the number of ECTS credits equivalent to a Level 9 or higher standard, relevant to the specialist area of care (equivalent to 60 ECTS or above), and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery 4th ed (2008). Cardiovascular Disease Integrated care prior to application* (See ** Note 1 below). AND (vii) Be required to demonstrate that they have continuing professional development (CPD) relevant to the specialist area. AND (viii) Have the ability to practice safely and effectively fulfilling his/her professional responsibility within his/her scope of practice. **Note 1: For Nurses/Midwives who express an interest in CNS/CMS roles and who currently hold a level 8 educational qualification in the specialist area (equivalent to 60 ECTS or above), this qualification will be recognised up to September 2026. And (b) Candidates must possess the requisite knowledge and ability, including a high standard of suitability and clinical, leadership, managerial and administrative capacity for the proper discharge of the duties of the office. 2. Annual Registration (i) Practitioners must maintain live annual registration on the appropriate/relevant Division of the register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann) for the role. And (ii) Practitioners must confirm annual registration with NMBI to the HSE by way of the annual Service user Safety Assurance Certificate (PSAC). 3. These positions will provide exciting opportunities for the successful candidate to develop their career by providing personal support services and community activation for adults with Autism. Your role will involve supporting and promoting each individual’s independence as well as enhancing their personal capacities. The successful candidates will assist in supporting individuals to develop and maintain their connections to other people and strengthen links to the local community Driving is a key duty and will be looked on favourably at application stage. Key duties associated with the role: · Support each person to effectively communicate and to ensure they make informed decisions, exercise choice, control and autonomy in their life. · Contribute to and monitor outcomes for the person as identified in the personal planning processes. · Identify supports and work in partnership with the individual, their family and other supports as outlined within individual plans · Work with Positive Behavioural Support plans providing consistent and skilled responses to behaviours that challenge Essential Criteria for Applicants: · Previous experience of supporting individuals presenting with Autism · Experience of promoting community inclusion and active participation. · Experience providing individuals with positive behaviour supports · Experience working with families, clinicians and other networks of support · Possess excellent communication skills · Have experience of implementing positive behaviour support programmes. · Have experience of a key working role, focusing on promoting independence and community integration. Essential Qualification required for Social Care Workers Hold one of the HSE approved qualification- see below if you don’t hold 1 of the below you will not be shortlisted. The successful Social Care Worker candidates must have the following Qualifications required · QQI Level 7 Bachelor of Arts in Applied Social Studies (Disability) - Open Training College; · Level 7 award in Social Care/Studies delivered by an Institute of Technology, TUD or National University of Ireland; · A qualification received outside the State recognised by NARIC as equivalent to the Irish Level 7 award. Salary Scale: Successful candidates will be paid in line with the June 2024 HSE revised consolidated Social Care Worker pay scale (point 1; €39,951– point 12; €56,089 per annum based on working a 39 hour week). Should you have no prior public sector experience you will be placed on point 1 of the pay-scale above.
