Details of Service Dublin South, Kildare and West Wicklow consists of 14 primary care networks. Each Network has Primary Care Teams who provide multidisciplinary services to distinct populations. These teams are based in Primary Care Centres located within the populations they serve Occupational therapists provide services to clients of all ages who live within the catchment area. Occupational therapists are key members of the primary care team and have opportunities to participate in integrated care in collaboration with our acute partners, voluntary agencies and community services across the CHO. The OT will be supported at all times by an experienced OT manager who will provide structured supervision, mentoring and clinical governance. The Occupational Therapy manager will support engagement in continuous professional development and opportunities for engagement in quality improvement initiatives. Primary Care Occupational Therapy Adult The model of service is mainly home based with interventions provided including falls prevention, cognitive rehabilitation, community skills training and self-care reablement. Occupational therapists also provide clinic-based services such as lifestyle redesign groups, health promotion groups, assistive technology training, complex seating assessments and upper limb rehabilitation. The primary focus is on helping the client to achieve their maximum potential in the areas of functional participation, health promotion and occupational wellness. Primary Care Occupational Therapy Paediatrics Occupational therapy is provided to children aged 0 – 18 years on a referral basis. The primary focus is to support children meet their maximum potential, through appropriate assessment and goal directed interventions. The model of service is clinic based and/or school based. Both individual and group interventions are provided as appropriate to best meet the child’s needs. Paediatric occupational therapy interventions are evidence based and could include fine and gross motor skills training, handwriting and sensory integration programmes. The occupational therapist would be expected to liaise with the Children’s Disability Network Teams (CDNT) and Children and Adolescent Mental Health Services (CAMHS) to ensure the needs of the children who live in the area are best met. The occupational therapist provides child centred interventions with all treatment planning based on the individual needs and goals of the child and their family. Integrated Care for Older Persons Senior Occupational Therapist (linked with Tallaght University Hospital, St James University Hospital, Naas General Hospital): The senior occupational therapist on the integrated older person’s service will work alongside a specialist multidisciplinary service primarily targeting and managing the complex care needs of the older person with multiple co-morbidities across a continuum of care.
